La maladie d'Alzheimer est une maladie affectant la vie du patient et de son entourage. L'aidant naturel est la personne de l'entourage venant en aide au proche malade pour les activités de la vie quotidienne. L'aide apportée au malade peut s'accompagner d'un fardeau et avoir des conséquences physiques, psychologiques, émotionnelles mais également sociales et financières sur l'aidant. Le pharmacien d'officine, acteur de santé de proximité peut intervenir afin d'améliorer la prise en charge de la dyade patient/aidant. Il peut également identifier une altération de l'état de santé de l'aidant et l'apparition d'un fardeau. Les objectifs de ce travail étaient (1) d'identifier les compétences et connaissances jugées nécessaires par les pharmaciens et préparateurs pour l'accompagnement de la dyade patient/aidant en officine ; et (2) d'évaluer la faisabilité du repérage du fardeau des proches aidants en officine et d'identifier les attentes des aidants vis-à-vis des pharmaciens. Pour cela, 2 études observationnelles ont été menées au sein de 10 officines à l'aide de 2 questionnaires à destination des pharmaciens/préparateurs et des aidants. Au total, 52 pharmaciens/préparateurs et 20 aidants ont participé à l'étude. Un besoin de formation concernant les aides humaines, financières, juridiques, matérielles ; les structures de répit et d'hébergement, et les associations a été identifié. Ce besoin de formation était en adéquation avec les attentes des aidants. Concernant le fardeau des aidants, 96% des pharmaciens/préparateurs et 70% des aidants ont déclaré que le pharmacien/préparateur pourrait avoir pour mission de repérer le fardeau en officine. En effet, l'étude a mis en évidence un fardeau chez 90% des aidants dont 50% à un stade modéré à sévère et 25% à un stade sévère. Ainsi en informant au mieux les aidants et en repérant précocement leur fardeau, le pharmacien contribuera à un meilleur accompagnement de la dyade patient/aidant.

Face aux millions de personnes touchées par les maladies d'Alzheimer et maladies apparentées, il est primordial que les pharmaciens d'officine, acteurs de santé de premier recours, puissent accompagner ces patients et ces proches aidants afin d'assurer un maintien à domicile optimisé. Pour que cette prise en charge soit la plus pertinente possible, il est important que les pharmaciens connaissent le parcours de soins de la maladie et les différents dispositifs d'aides (financières, juridiques, matérielles et humaines) auxquels les patients et leurs proches aidants peuvent recourir. Dans cet objectif, une revue bibliographique a été effectuée afin de décrire le parcours de soins de la maladie d'Alzheimer et maladies apparentées et l'ensemble des structures et des aides destinées à ces personnes. La partie expérimentale a permis l'élaboration de deux outils synthétiques et pratiques à destination des équipes officinales. Ces outils ont pour objectif de synthétiser les différentes étapes du parcours de soins et de mettre à disposition des informations sur les principaux dispositifs et structures vers lesquelles l'équipe officinale peut orienter le patient et son proche aidant. Ces outils ont été développés sous forme de posters à afficher dans les pharmacies d'officine, après validation d'un groupe de travail hospitalier et officinal. Ces derniers permettront donc aux équipes officinales d'avoir à disposition les informations nécessaires pour orienter et accompagner au mieux les patients atteints de la maladie d'Alzheimer et maladies apparentées et leurs proches aidants.
