59 patients de plus de 65 ans traités pour fracture de l’extrémité distale de l’humérus entre 2000 et 2010 ont été revus de manière rétrospective. 46 patients ont été revus cliniquement et 13 à partir des données récupérées dans les dossiers médicaux au recul minimum de 6 mois. Il existait 50 femmes pour 9 hommes de 78,3 ans de moyenne d’âge. Selon la classification de l’AO, il existait 37% de fractures de type A, 14% de type B, et 49% de type C. Le traitement se répartissait en : 5 traitements fonctionnels, 33 ostéosynthèses et 21 arthroplasties. Au recul moyen de 3,1 ans le score MEPS moyen était de 87,2 points/100, le score QuickDASH moyen de 22,44 points, l’arc de mobilité moyen de 95,6 degrés. Il existait 86% de complications, et 10% de reprise chirurgicale. Pour les patients de plus de 80 ans traités pour fracture type C, le score MEPS était supérieur, de façon significative (p=0.03), dans le groupe arthroplastie par rapport au groupe ostéosynthèse.
