Core body temperature is an important physiological parameter used to identify whether a patient displays a normal, hypothermic, or hyperthermic state. It is routinely monitored during cardiac surgeries and general anesthesia. Currently, the most effective methods for measuring core body temperature are also the most invasive. While select devices have been designed to enable surface recording of internal temperature, none have been implemented in U.S.-based hospitals. The objective of this study was to create a noninvasive core temperature sensor and evaluate its potential of becoming a widely used clinical tool. In tissue phantom and human-based experiments, the prototype performed effectively and posed no safety risk. Provided the prototype can be successfully translated into a more streamlined medical device, it stands to become a staple in operating rooms around the nation.

The rapid, noninvasive measurement of core temperature in humans is desireable since both elevated core temperature (hyperthermia) and depressed core temperature, including oral and rectal thermometry, are unacceptable for field core estimation because of both physical and cultural problems. A rapid, noninvasive means of determining core temperature would be beneficial for in field management of combat casualties, as well as in peace-time evaluations of military materiel and physical workload assessments. In this Phase I SBIR effort, we have identified a new approach to rapidly and noninvasively measuring core temperature. We have established a relationship between exhaled air temperature, effective ambient temperature, and core temperature. In this relationship, the effective ambient temperature is the ambient air temperature corrected for wind velocity (wind chill), humidity, and air density. The results of our Phase I experiments indicate, that it is feasible to determine core temperature can be determined within 0.5 C of the temperature measured by an ingestible thermometry pill.

Phase I development used a fast rise-time thermocouple to monitor the temperature of the expired air of an anesthetized animal model and investigate which parameters most greatly affect the expired air temperature at various stages of hypothermia. The Phase I goal of testing a working prototype has been achieved in which the critical parameters that influence the expired air temperature have been determined and controlled such that under most experimental conditions the error in estimation of core temperature is less than 1 C over a range of hypothermic temperatures. The Phase II goal will be development of a portable unit suitable to measure core temperature under field conditions. The proposed system will enhance the ability of the military to operate in low temperature environments by being able to monitor whether their troops are becoming hypothermic. It will be able to monitor temperature quickly in a noninvasive manner without the removal of clothing.
