Excerpt from The Medical Clinics of Chicago, Vol. 2: November, 1916 In addition to the pain and vomiting he has complained at times of sour, acid eructations, of burning throat pain, and of burning, dull depressing pain in the upper abdomen, relieved by food, hot water, or soda-mint tablets. He thinks that his stool at times has been black, but that this has disappeared again in a few days. At the time of the black stools he has not noticed any untoward symptoms, such as weakness, fatigue, vertigo, etc. He has lost moderately in weight, about I 5 pounds, during the five years of his illness. His past history is essentially negative, with the exception that in 1914, two years ago, he suffered with swollen knees, at which time his physician diagnosed his condition as gout. He denies all venereal diseases. Family History - He was married while quite young. His wife is well and living. He has two boys, one eleven and the other nine years of age, both well. His wife has had no miscarriages. His father 13 living and well. His mother, he thinks, died of abdominal carcinoma. There are no chronic constitutional dis eases ih his family. Habits - He smokes cigarettes quite considerably recently. Formerly he smoked cigars and a pipe, but in moderation. He drinks beer occasionally, although practically none during the past year. He feels that alcoholic drinks make his stomach complaints worse, and during the exacerbations of his symptoms he never takes alcohol. Excerpt from The Medical Clinics of Chicago, Vol. 1: November, 1915; Number 3 I saw a great many more cases of typhoid as an intern, in Cincinnati, than I do now, and I made up my mind that when I got through my apprenticeship I would do something besides feed patients on milk alone. Little by little physicians became more liberal in their diet, but the majority of men up to five or six years ago kept their patients principally upon milk, giving them perhaps other uids. This was a little better, but during the last four or five years the fashion has swung around the other way. Men are recommending food in quantities - a good deal more than the ordinary diet contains. A man will recommend as much as 4000 or 4500, or even 5000 calories of food a day, which is as much as a man doing the hardest kind of work can consume. I have always opposed the simple milk diet and the simple uid diet, but I am just as much against the overfeeding. I see no reason why a patient that is running a fever should be fed more than he would if he were up and about. I have never put a patient on a pure uid diet, but I don't believe in overfeeding either. In the first place, I think a diet consisting of about 2800 or 3000 calories is just about right. That is just about what most of us consume in a day. Is it necessary to keep the typhoid patients on a liquid diet? Emphatically No. The idea was that by keeping the patient on a strictly milk diet the fecal residue would be less, but that is not true. Excerpt from The Medical Clinics of Chicago, Vol. 1: January, 1916 It is Of considerable importance to know that experimentally it has not been possible to produce a cerebrospinal meningitis by infecting the nasal cavities. The question then is, What significance are we to place upon the presence Of the organisms in the nasal secretions? In epidemics, and particularly during the last few years, the systematic examination Of the nasal secre tions has revealed the fact that 65 to 80 per cent. Of the individuals coming in contact with cases Of epidemic cerebrospinal meningitis contain the organism in their nasal secretions. In other words, they are carriers. Only a small percentage of these carriers actually develop the disease, but they act as distributors of the disease, and from them susceptible individuals are actually infected. So far as can be determined, the meningeal infection prob ably occurs through the blood; 30 to 40 per cent. Of the clinical cases of epidemic meningitis will give a positive blood - culture. When the methods Of examination are perfected it may be pos sible to demonstrate a higher percentage of positive blood cultures. The bacteria are carried to the meninges in the blood, and there produce the clinical manifestations Of the disease. In the experimental work it has not been possible to produce a meningitis by nasal sprays, by subcutaneous or intravenous injections. It is necessary to make a direct infection of the meninges. When the basal meninges become involved, in the course Of two, three, or four days, the organism appears in the nasal secretion. Whether this is a means of elimination or not is not definitely known.
