During the past two decades research on the testis has created a vast number of publications. New techniques, such as cell and gene clon ing, germ cell manipulation and transplantation, as well as gene ex pression, have surpassed the classical approaches that involved studies on receptor activation and endocrine, paracrine and autocrine regula tory mechanisms. Due to the expansion of this field of research it has become increasingly difficult to integrate the large number of observa tions and relate them to the physiology and pathophysiology of the tes tis. More emphasis must, therefore, be put on the integration of isolated findings and the construction of the total picture. In order to stimulate this specific mental activity we have named this book Testicular Tan gram. We hope that the chapters in this book will be used as pieces of the testicular puzzle and that the reader enjoys composing new shapes. The chapters of this book are representative of the plenary lectures presented at the I ih European Workshop on Molecular and Cellular Endocrinology of the Testis held in Doorwerth The Netherlands, from April 6 - I 0, 2002. Unwanted childlessness affects approximately one in six couples worldwide. - though the exact proportion of the predominant cause of the problem remains controversial, according to the World Health Organization (WHO), in nearly 40% of cases the cause can be attributed to the female, in 20% to the male, in 25% to both, and in 15% the cause remains unknown. Based on these ?gures, the - cidence of male factor infertility in the general population is approximately 7%. The majority of these men, approximately 30%, experience irreversible idiopathic infertility and cannot father children without some form of medical intervention. Male factor infertility, in addition, may be caused by testicular germ cell cancer, which is known to represent the most common cancer among young men, aged 15 to 35 years, in Western industrialized countries. The number of affected men has increased dramatically over the past 50 years. There is now growing evidence that human testicular germ cell cancer originates from fetal germ cells exhibiting an aberrant programme of gene expression, and tumour progression may be favoured by an aberrant Sertoli cell-germ cell communication.

Personalized medicine employing patient-based tailor-made therapeutic drugs is taking over treatment paradigms in a variety of ?elds in oncology and the central nervous system. The success of such therapies is mainly dependent on ef?cacious therapeutic drugs and a selective imaging probe for identi?cation of potential responders as well as therapy monitoring for an early bene?t assessment. Molecular imaging (MI) is based on the selective and speci?c interaction of a molecular probe with a biological target which is visualized through nuclear, magnetic resonance, near infrared or other methods. Therefore it is the method of choice for patient selection and therapy monitoring as well as for speci?c e- point monitoring in modern drug development. PET (positron emitting tomography), a nuclear medical imaging modality, is ideally suited to produce three-dimensional images of various targets or processes. The rapidly increasing demand for highly selective probes for MI strongly pushes the development of new PET tracers and PET chemistry. ‘PET chemistry’ can be de?ned as the study of positron-emitting compounds regarding their synthesis, structure, composition, reactivity, nuclear properties and processes and their properties in natural and - natural environments.
