L'insomnie est un trouble du sommeil tres frequent et son traitement est un veritable enjeu de Sante Publique. Elle est difficile a traiter car elle peut avoir diverses origines: pathologique (psychologique, liee a une pathologie neuronale...) et physiologique chez la personne agee. Il existe differents moyens de diagnostic pour en connaitre l'etiologie mais ceux-ci ne sont pas souvent utilises car ils demandent du temps et des examens multiples dans des centres du sommeil trop peu nombreux. La reponse aux plaintes d'insomnie est le plus souvent pharmacologique; les traitements par hypnotiques sont instaures de maniere chronique et peuvent engendrer de nombreux effets indesirables ayant un cout pour l'Assurance Maladie. C'est pour cette raison que de nombreux organismes de sante se sont interesses a cette question pour mieux connaitre les pratiques medicales. L'etude en region Champagne-Ardenne a ete realisee afin de mettre la personne agee au centre des preoccupations et rendre cette etude plus humaine qu'avec de simples banques de donnees.

L'insomnie est la plainte la plus fréquemment rencontrée à l'officine parmi les troubles du sommeil. Afin de palier à ce trouble, les hypnotiques se présentent comme une solution efficace pour faciliter l'endormissement et maintenir le sommeil. Ils sont ainsi largement prescrits et leur consommation augmente avec l'âge. En effet, la France se positionne au 3ème rang parmi les pays d'Europe les plus consommateurs d'hypnotiques. Cependant, malgré les recommandations émises par les autorités de santé dans le but de promouvoir le bon usage et limiter leurs prescriptions, ils constituent un véritable problème de santé publique quant à leur durée d'utilisation et les effets indésirables qu'ils comportent (somnolence, chute, accident, déclin cognitif, dépendance). Ces effets indésirables peuvent avoir des conséquences préjudiciables notamment chez le sujet âgé à haut risque d'iatrogénie en raison de la polymédication et des modifications physiologiques qui impactent les paramètres pharmacocinétiques. Le pharmacien a donc un rôle déterminant dans la détection des prescriptions inappropriées et le respect du bon usage du médicament. Cette thèse comporte une partie bibliographique suivie d'une étude officinale dont l'objectif est d'évaluer si les prescriptions d'hypnotiques chez les personnes de plus de 65 ans sont appropriées et conformes aux recommandations. Les résultats de cette étude serviront de base pour rappeler les conseils à délivrer lors de la délivrance d'hypnotiques et les alternatives à proposer en première intention pour prendre en charge l'insomnie du sujet âgé.

Trop de troubles mineurs du sommeil sont systématiquement qualifiés d'insomnie entraînant un abus de prescriptions d'hypnotiques injustifiées, se traduisant par des effets secondaires graves très fréquents chez le sujet âgé : troubles de la conscience, chutes, fractures, démence, rapport probable avec la maladie d'ALZHEIMER. Seules 10 à 20 % des plaintes du sommeil sont de véritables insomnies. En 2012, 7 millions de français ont consommé une benzodiazépine hypnotique. Il faudra tenir compte des modifications physiologiques liées à l'âge, influençant le devenir du médicament dans l'organisme, et rester vigilant face à toute prescription d'hypnotiques chez le sujet âgé. Devant toute plainte de mauvais sommeil chez un sujet âgé, la priorité devrait être la recherche de la cause pouvant provenir d'un non-respect des règles hygiéno-diététiques. Les benzodiazépines à demi-vie courte, seront réservées uniquement aux insomnies les plus sévères, toujours sur une durée de traitement très brève.
