Le patient âgé est exposé à un risque accru de mortalité et d'événements indésirables notamment de chutes, d'iatrogénie, d'hospitalisations répétées et d'institutionnalisation. L'objectif de notre étude est de mettre en place et d'évaluer le déploiement de la pharmacie clinique en médecine gériatrique en priorisant les patients les plus fragiles et de détailler les missions ayant pour objectif de renforcer le rôle du pharmacien en tant que professionnel de santé dans le parcours de soins du patient. Notre étude montre que les patients âgés très fragiles sont majoritairement sujets à la polymédication. La complexité du parcours de santé des personnes âgées fragiles et ses conséquences appellent à une coordination des acteurs de la ville et de l'hôpital. Cette coordination suppose des compétences, des connaissances et une attention spécifique envers les patients. Ainsi, les activités diverses du pharmacien contribuent à sécuriser le parcours de ces patients à risque. Les perspectives seraient d'étendre nos activités en mettant en place d'avantage de conseil pharmaceutique ainsi que de renforcer le lien ville/hôpital.

En France, on compte 13,4 millions de personnes âgées de plus de 65 ans soit 20% de la population en 2019 d'après l'INSEE. Les personnes âgées représentent une population fragile en raison de leur âge avancé et leurs pathologies multiples. Naturellement, la polymédication est souvent présente chez les séniors pour traiter leurs différentes pathologies. Du fait de la polymédication et d'un parcours de soin complexe, cette population est très exposée aux évènements iatrogènes. La iatrogénie médicamenteuse représente un enjeu majeur de santé publique chez le sujet âgé. Les Établissements d'Hébergement Pour Personnes Âgées Dépendantes (EHPAD) sont un lieu de vie de ces personnes âgées fragiles et en perte d'autonomie. Nous nous sommes intéressés à l'organisation du circuit médicamenteux au sein de l'EHPAD où le risque d'évènements iatrogènes est plus élevé en raison de la multiplicité d'acteurs intervenant auprès des résidents et d'une population gériatrique fragile. La sécurisation du circuit médicamenteux en EHPAD est ainsi essentielle dans la qualité de prise en charge des résidents. Les EHPAD ont aujourd'hui la possibilité de signer des conventions avec les pharmacies d'officine pour la mise en pilulier des traitements des résidents, le pharmacien d'officine a donc un rôle majeur dans la qualité et l'optimisation des traitements des résidents. La loi HPST (Hôpital, Patient, Santé et Territoire) en 2009 a introduit une nouvelle mission pour les pharmaciens d'officine en convention avec des EHPAD ; celle du pharmacien référent. En effet, le pharmacien est un acteur incontournable du circuit médicamenteux de par ses connaissances pharmacologiques et cliniques. Le pharmacien référent a globalement pour rôle de s'assurer de la qualité et de la sécurité de toutes les étapes du circuit du médicament et de participer à la prise en charge des résidents. Bien que ce rôle de pharmacien référent ait permis de sécuriser le circuit du médicament sur plusieurs points, il reste plusieurs pistes d'amélioration pour la prévention de la iatrogénie médicamenteuse.
