Objectifs : étudier l'épidémiologie, les circonstances et les facteurs de risque de chute après un AVC pour les patients hospitalisés en neurologie ou en rééducation, afin d'aider à la réalisation de programmes de prévention efficaces. Méthode : réalisation d'une revue systématique de la littérature, en anglais et en français, de 1995 à 2019, qui étudie la survenue des chutes en hospitalisation chez les patients majeurs ayant fait un Accident Vasculaire Cérébral. Les recherches ont été menées via les moteurs de recherche Pubmed, Web of Sciences, et EM premium, en utilisant les mots clefs « AVC », « accident vasculaire cérébral », « chute », « stroke », « accidental falls ». Les articles inclus étaient des études observationnelles transversales, de cohorte ou rétrospectives analysant les facteurs de risque de chute post-AVC à l'Hôpital. Nous avons extrait les données disponibles sur l'âge moyen des patients, la taille de l'échantillon, les incidences, circonstances, conséquences des chutes, les différents facteurs de risque étudiés et les éventuelles limites de chaque étude. Résultats : 20 articles ont été retenus, ils concernaient les patients en service de neurologie pour 3 d'entre eux, les autres concernaient les patients en service de rééducation. Les proportions de patients chuteurs ou les incidences des chutes sont variables. La proportion de patients chuteurs est comprise entre 3.3% et 49%. Les chutes surviennent principalement dans la chambre du patient, ou la salle de bain, lors des transferts. Les conséquences lésionnelles graves sont rares. Les éventuelles conséquences psychiques n'ont pas été analysées. Les facteurs de risque étudiés sont très variables. Les troubles de l'équilibre, les troubles cognitifs, la perte d'indépendance fonctionnelle globale, et l'héminégligence sont des facteurs de risque de chute récurrents. Les méthodes d'évaluation des différents facteurs de risque potentiels sont différentes d'une étude à l'autre. Le rôle de certains paramètres, comme les troubles moteurs, est moins clair. Conclusion : la prévention des chutes doit se concentrer sur les activités à risque comme les transferts des patients, principalement dans sa propre chambre ou la salle de bain. Il existe un intérêt probable à l'évaluation des troubles de l'équilibre, de la présence d'une héminégligence, de troubles cognitifs, et l'évaluation globale de la perte d'indépendance fonctionnelle pour cibler les patients à risque de chute. La réalisation d'études complémentaires est nécessaire pour confirmer ces résultats, et clarifier les données que nous avons à disposition sur les différents facteurs de risque potentiels.
