Pour parvenir au bon diagnostic, la démarche du vétérinaire se découpe en plusieurs étapes, que sont, un recueil précis des commémoratifs et de l'anamnèse, un examen clinique, lui même plus ou moins complété par un examen du système nerveux et de l'appareil locomoteur. Selon ses hypothèses diagnostiques hiérarchisées, la motivation des propriétaires et les moyens disponibles dans sa structure, le clinicien choisira spécifiquement un ou plusieurs examens complémentaires parmi les suivants : analyses sanguine, urinaire et de liquide cérébro-spinal, biopsies musculaires et nerveuses, electromyo-neurographie ainsi que les différentes techniques d'imagerie médicale accessibles en médecine équine. Ce travail présente les principales maladies nerveuses, musculaires et vasculaire responsables de troubles de la démarche chez le cheval, leurs facteurs de risque, leur(s) étiologie(s) suspectée(s), les principaux signes cliniques qu'elles engendrent, les examens complémentaires pertinents ainsi que leur approche thérapeutique. Selon l'affection considérée, le traitement comportera un volet médical et/ou chirurgical, s'accompagnant de pronostics vital et sportif allant de sombre à favorable. Afin d'éclairer correctement le propriétaire quant aux pronostics et de mettre en place le traitement spécifique adéquat, il est donc primordial pour le vétérinaire d'envisager les bonnes hypothèses diagnostiques. Ainsi, l'objectif de cette thèse est de montrer l'intérêt pour le praticien généraliste équin de faire également appel à ses connaissances en médecine interne et non seulement à celle de l'appareil ostéo-articulaire face à un cheval présenté pour trouble de la démarche.

Spitfire pilot Ross Smith Stagg was one of 33 Allied airmen to defend Darwin against Japanese invasion on May 2, 1943. As one of 14 pilots shot down or experience mechanical failure in the ensuing battle, he parachuted into the sea 18 km from land, 100 km southwest of Darwin in the Fogg Bay area. He reached the shore in a dinghy. For the next 15 days he trudged through inhospitable country in a futile attempt to return to Strauss airbase. What should have been a few days walk turned into his worst possible nightmare as he stumbled aimlessly through mosquito and crocodile infested swamps. "It was almost six days I'd been without sleep, apart from a short period of unconsciousness and those few moments before I fell out of that tree," he said. " I became demented by the cavalcade of mosquitoes and hallucinating badly". His experience was only to worsen - he waded halfway across a tidal river to be confronted by a large saltie. Darwin historian John Haslett help Stagg map the original route by retracing his steps, even managing to relocate an American Kittyhawk Stagg found crashed in the middle of nowhere.
