Les services d'accueil des urgences sont de plus en plus confrontés à la prise en charge des personnes âgées. L'aval des urgences, pour la population gériatrique fragile, est assuré majoritairement par le service de Médecine Interne (du fait de sa polyvalence) et par l'unité de courte durée d'hospitalisation (UHCD) des urgences. A l'hôpital Saint-Antoine, une unité d'évaluation gériatrique (UG) ou Post-Porte, crée en Octobre 1999 au sein de l'UHCD, permet d'orienter les sujets âgés dans la filière de soins gériatriques adaptée à leur état de santé. En 2000, l'orientation des patients âgés, admis à l'issue de la consultation des urgences de l'hôpital Saint-Antoine, se fait pour 37% vers l'UHCD, 7% vers le Post-Porte et 7% vers le service de Médecine interne. Ainsi, 51% des personnes âgées nécessitant une prise en charge médicale hospitalière sont transférés dans ces services d'aval des urgences. L'objectif de l'étude, rétrospective de cohorte, est de comparer les durées de séjour de la population âgée de 80 ans et plus, dans l'UG et dans le service de Médecine interne, au cours de l'année 2000. Les informations recueillies concernaient les données socio-démographiques des patients, leur diagnostic à l'issu de l'hospitalisation (les pathologies d'organes - ou les accidents vasculaires cérébraux et les démences ont été isolés, les symptômes mal définis et la perte d'autonomie), leur mode de vie ainsi que leur durée moyenne de séjour (DMS). Les résultats montrent que l'orientation prédominante des sujets âgés en Médecine interne est un retour à domicile pour 54% d'entre eux, et dans l'UG un transfert en milieu gériatrique pour 43% d'entre eux...
