This is why I have chosen the country to compare especially the Health Care System with the German one. Not only the economic achievements are comparable, but first of all the Demographic Change. In 2020 28% of the Japanese population will be over 65 years. In Germany it is prognosticated to be around 21% (Tab.1). Moreover the Japanese Health Care System is known as own of the cheapest of the industrialized countries. This leads to the question of adoption of some parts or ideas from the Japanese system to the German one. In my elaboration I want to describe a case of a family in Japan regarding the family and work situation, the Health Care System in general and additionally the system of taking care of the elderly and the children. My example family has got following parts: The mother is 42 years old, she works as a nurse in an outpatient department of a local government. In addition she has to take care of her parents almost every day. The father is 45 years old, works as an engineer and likes his hobby, which is driving motorbikes. The daughter of them is 7 and their son is 13, both go to school. The grandmother has got dementia, she is 76 years old and lives together with her husband, who is 76 and has got diabetes. They live in the neighborhood of their children and grandchildren. Case 1: Values, culture, roles of the family members. Case 2: Health Care System of Japan in comparison to the German one. What happens after a traffic accident of the father? Case 3: Elderly and Child Care in general and in the case of the accident and depression of the mother.

In recent years, Japan and Germany have been facing very similar challenges: aging populations, changing employment structures, long-lasting economic stagnation, and globalization, all of which threaten existing social security arrangements. Both countries are in a number of respects more socially and politically regulated, and in this sense less liberal, than the Anglo-American economies. Nonetheless, during the last couple of years several important social policy reforms have been implemented. A comparison of these reforms may provide valuable insights into the changing character of "conservative" welfare states. This volume concentrates on two fields of social policy: long-term care insurance and public pensions. These social insurances are at the center of current public debate in both countries because population aging translates immediately into a higher demand for care for the elderly and old age security. The contributions range from demographic and policy implications of aging through detailed analyses on the different reform measures to specific aspects such as bioethical or regional policy considerations. This open access book examines the triangle between family, gender, and health in Europe from a demographic perspective. It helps to understand patterns and trends in each of the three components separately, as well as their interdependencies. It overcomes the widely observable specialization in demographic research, which usually involves researchers studying either family or fertility processes or focusing on health and mortality. Coverage looks at new family and partnership forms among the young and middle-aged, their relationship with health, and the pathways through which they act. During the last 25 years, life expectancy at age 50 in the United States has been rising, but at a slower pace than in many other high-income countries, such as Japan and Australia. This difference is particularly notable given that the United States spends more on health care than any other nation. Concerned about this divergence, the National Institute on Aging asked the National Research Council to examine evidence on its possible causes. According to Explaining Divergent Levels of Longevity in High-Income Countries, the nation's history of heavy smoking is a major reason why lifespans in the United States fall short of those in many other high-income nations. Evidence suggests that current obesity levels play a substantial part as well. The book reports that lack of universal access to health care in the U.S. also has increased mortality and reduced life expectancy, though this is a less significant factor for those over age 65 because of Medicare access. For the main causes of death at older ages-cancer and cardiovascular disease-available indicators do not suggest that the U.S. health care system is failing to prevent deaths that would be averted elsewhere. In fact, cancer detection and survival appear to be better in the U.S. than in most other high-income nations, and survival rates following a heart attack also are favorable. Explaining Divergent Levels of Longevity in High-Income Countries identifies many gaps in research. For instance, while lung cancer deaths are a reliable marker of the damage from smoking, no clear-cut marker exists for obesity, physical inactivity, social integration, or other risks considered in this book. Moreover, evaluation of these risk factors is based on observational studies, which-unlike randomized controlled trials-are subject to many biases.
