Introduction : La promotion de l'allaitement maternel (AM) fait partie des objectifs de santé publique du Programme National Nutrition Santé (PNNS) de 2019-2023. Le Pas-de-Calais est l'un des départements français où le taux d'initiation à l'allaitement est le plus bas (43,8 % en 2019 vs 66,7 % pour la métropole). Quels sont les facteurs associés à l'initiation de l'allaitement dans ce département ? Matériel et méthode : Enquête épidémiologique descriptive à partir des données des certificats de santé du premier certificat de santé. Nous avons analysé le lien avec l'évolution de la densité de médecins généralistes de 2014 à 2019. Résultats : L'initiation de l'allaitement dans le Pas-de-Calais a augmenté (39 % en 2014 à 42 % en 2019). En parallèle, la densité de médecins généralistes a diminué dans le département (10,3 pour 10 000 habitants en 2014 à 9,5 en 2019) ; il n'y avait pas de corrélation entre ces deux variables. L'initiation de l'allaitement était plus fréquente chez les mères âgées d'au moins 25 ans, qui travaillent ou dont le conjoint travaille, qui sont cadres ou de profession intermédiaire et qui avaient suivi des cours de préparation à la naissance. L'accouchement par voie basse, un bébé né à terme et de poids normal avaient aussi une influence positive sur l'initiation de l'allaitement . Conclusion : Pour augmenter le taux d'allaitement dans le Pas-de-Calais il faut prendre en compte, dès le début de la grossesse, les caractéristiques socio-démographiques des familles, en particulier pour les femmes les plus jeunes et inactives.
