Les allergies rapportées aux médicaments sont relativement fréquentes dans la population générale, mais leur prévalence, leurs caractéristiques et leur impact tout médicament confondu chez les patients hospitalisés sont peu connus. Notre étude prospective a pour but de décrire ces allergies rapportées par les patients hospitalisés et d'étudier leur impact sur la prise en charge hospitalière. Chaque patient hospitalisé dans le service de médecine interne rapportant une allergie a été inclus, sur une période de six mois. Le recueil de données s'est fait via un questionnaire sur les caractéristiques de l'allergie. L'impact sur la prise en charge était complété par le médecin prescripteur. Ces patients ont ensuite été comparés aux autres patients hospitalisés ne déclarant pas d'allergie. Nous avons inclus 58 patients avec un total de 92 allergies déclarées. Le nombre moyen d'allergies déclarées par patient est de 1,59, et la moyenne d'âge de 72,24 ans. Nous avons observé une prévalence de 5,6% d'allergies rapportées dans le service de médecine interne concernant en majorité les anti-infectieux, avec des réactions initiales cutanées. 29.3% ont été jugés utiles à la prise en charge du patient. On retrouvait une différence significative pour le sexe en faveur du sexe féminin. Il n'y avait pas de différence pour l'âge, la durée d'hospitalisation et le nombre de passages en réanimation. La mise en place d'outils simples pour éliminer une allergie médicamenteuse semble nécessaire afin de limiter son impact sur la prise en charge, notamment sur la prescription d'alternative médicamenteuse pourvoyeur de iatrogénie, d'antibiorésistance, ou de majoration de coût.

Les allergies médicamenteuses, dont les plus fréquentes les toxidermies, représentent un enjeu majeur de santé publique. Il existe bien souvent une errance diagnostique concernant ces allergies rapportées, avec un lourd retentissement médico-économique. L'objectif de notre étude était de déterminer la fréquence des allergies médicamenteuses, antérieures à leur hospitalisation, déclarée par les patients et de les classer en allergie vraie documentée, probable, ou peu probable avec une réaction allergique. Il s'agissait d'une étude descriptive monocentrique rétrospective. Tous les patients du service de dermatologie d'hospitalisation longue durée du CHU de Caen étaient interrogés sur la question de l'allergie médicamenteuse dans leurs antécédents médicaux, à partir d'un questionnaire standardisé, de mai 2014 à mars 2015 ; le questionnaire était également soumis au médecin traitant. Les patients déclarant une allergie étaient alors classés en 3 groupes : 1) allergie vraie documentée, 2) allergie probable et 3) allergie peu probable. Sur 693 patients interrogés, 100 patients (14,4%) se déclaraient allergiques à un/des médicament(s). 2 avaient une allergie vraie (1,3%), 72 une allergie probable (45,8%) et 83 une allergie peu probable (52,9%). 5,1% seulement avaient eu des tests allergologiques. L'allergie déclarée était considérée comme peu probable dans plus de la moitié des cas. Il est primordial d'améliorer la prise en charge diagnostique des allergies médicamenteuses par une documentation exhaustive des allergies par le médecin traitant, par une orientation allergologique spécifique et par le port obligatoire d'une carte d'allergie informative.

Until the 1990s, it was generally accepted that medicines were first developed for adults and their use in children was investigated later, if at all. One of the main tasks of hospital pharmacies was the manufacturing of child-appropriate formulations in a more or less makeshift way. The first change came in 1997 with U.S. legislation that rewarded manufacturers to do voluntary pediatric research. Ten years later, the European Union passed legislation that required manufacturers to discuss all pediatric aspects, including formulations, with the regulatory authorities as a condition of starting the registration procedure. In consequence, manufacturers must now cover all age groups, including the youngest ones. So far, pediatric formulations were more a focus for academic researchers.
