Introduction : La couverture vaccinale pour le BCG reste insuffisante en Ile-de-France depuis la suspension de l’obligation vaccinale. Les facteurs influençant la décision de vaccination sont multiples. Cette étude cherche à mesurer l’impact de la suspension de l’obligation vaccinale en précisant le rôle des patients dans cette décision. Matériel et méthode : Il s’agit d’une étude prospective, multicentrique réalisée auprès des patients majeurs de trois cabinets de médecine générale en Ile-de-France (Paris 10e, La Courneuve, et Gagny) en 2013. Au total, 198 sujets ont été inclus, par questionnaires anonymes. Le critère de jugement principal était l’intention de faire vacciner un potentiel futur enfant. Les critères de jugement secondaires étaient le médecin qui pratiquerait la vaccination, si un retour à l’obligation vaccinale leur semblait nécessaire en Ile-de-France, si la suspension de l’obligation vaccinale les avait fait changer d’avis sur le BCG, et s’ils étaient suffisamment informés sur la tuberculose et le BCG. Une analyse statistique multivariée a été effectuée.

Introduction : La tuberculose est une maladie présente dans le monde mais la France fait partie des pays à faible incidence. C’est pourquoi la politique vaccinale a été modifiée en 2007 qui a suspendu l’obligation vaccinale par une vaccination dite ciblée. Les enfants de Seine et Marne sont concernés par cette vaccination car ils sont considérés à risque élevé comme tous les habitant de l’Ile de France. Matériel et Méthode : Nous avons interrogé 300 médecins généralistes libéraux installés en Seine et Marne par courrier à l’aide d’un questionnaire afin de connaître leurs pratiques ainsi que les déterminants vis-à-vis de la vaccination BCG. Cette enquête a eu lieu entre le 30 mars 2013 et le 30 juin 2013 : 130 médecins ont répondu. Résultats : 85% des médecins réalisent actuellement la vaccination BCG. Ceux qui connaissent les recommandations officielles actuelles et ceux qui vaccinaient avant 2007 réalisent plus la vaccination. Les médecins ne réalisant pas la vaccination estiment rencontrer plus fréquemment des complications à la vaccination et adressent plus fréquemment les enfants à un confrère pour réaliser la vaccination. Les principaux freins évoqués sont la complexité de la réalisation de l’injection intradermique, la disparition du Monovax® ainsi que les effets secondaires et les complications à la vaccination. Conclusion : Près de 75% des enfants Seine et Marnais sont vaccinés par le BCG. Afin d’améliorer ce taux de vaccination, il serait utile de renforcer la communication autour de la politique vaccinale (utilité du vaccin, ses limites, la population ciblée, les techniques d’injections, les effets normaux attendus suite a la vaccination mais également les complications etc...) par le biais des autorités sanitaires, des médias, de la formation des futurs professionnels de santé et de l'enseignement postuniversitaire.
