" Tobacco use remains the leading preventable cause of death, disease, and disability in the United States. There is an expectation that clinicians address every patient who uses tobacco and offers them a smoking cessation treatment. Home health care providers have an excellent opportunity to help facilitate smoking cessation with patients in the community. This evidence-based project aimed to increase the knowledge of the home healthcare providers on brief smoking cessation counseling using the 5A's model (ask, advise, assess, assist, and arrange) and increase compliance with providers offering smoking cessation counseling to community patients. The educational project took place at a home healthcare agency that provides care for home-bound patients. The participants (N = 24) included home health nurses, home health attendants, aides, and agency office staff. Outcomes were smoking cessation knowledge and compliance with smoking cessation counseling of patients. The participants were educated on smoking cessation counseling, and the nurse participants were coached to offer smoking cessation counseling to patients. A pre/post knowledge of smoking cessation test and a 4- item Likert evaluation survey were used to measure outcomes. Data were analyzed using descriptive statistics and percent change calculations. Participant means knowledge on smoking cessation increased 38.7%, exceeding the 20% benchmark. The nurses were highly likely to continue using the 5A's model in counseling patients on smoking cessation after the project implementation based on the evaluation survey. Recommendations include incorporating a smoking cessation counseling protocol at the site to sustain the change and improve opportunities for patients to quit smoking in the community. Tobacco use represents a significant cause of morbidity and mortality in the U.S. and is a contributing risk factor for multiple pulmonary, cardiovascular, and oncologic diseases. Despite documented evidence of these health hazards, use of tobacco products remains high, particularly in rural America. While providers may frequently question patients regarding tobacco use and inform patients of associated health hazards, a standardized, structured process for delivering smoking cessation counseling (SCC) is rarely established in practice settings. One such method for standardization of SCC is the 5A’s (Ask, Advise, Assess, Assist, Arrange) model which has a robust body of evidence to support its efficacy. An evidence-based practice project to incorporate evidence-based SCC into practice at a clinic in rural Virginia was performed. Provider education concerning the 5A’s of SCC was delivered to all clinic providers along with educational handouts. Data collection involved baseline and postintervention retrospective chart review of provider rate of SCC performance and the percentage of patients who smoke cigarettes at the clinic. An increase in provider performance of SCC and a decrease in current smokers was identified on evaluation of 90-day postintervention data. Results suggest that the evidence-based intervention was successful at this clinic in producing desired outcomes, and other clinics may find this example of incorporating evidence-based SCC into practice helpful.

"Tobacco smoking is a major public health problem and the leading cause of disability both in the United States and the rest of the world (Reitsma et al., 2017). The purpose of this evidence-based project (EBP) was to reduce tobacco use among psychiatric patients through the routine utilization of the 5 A's smoking cessation model by healthcare providers. The problem identified was described as increased tobacco use among psychiatric patients. The literature that supported the 5 A's model of tobacco cessation (ask, advise, assess, assist, and arrange) is an evidence-based tool created by the World Health Organization (2014). The theory and EBP model that offered guidance to the EBP project were the Transtheoretical Model of Health Behavior Change (TTM) and the IOWA model respectively. The project was implemented at a psychiatric outpatient clinic site. The intervention included education of clinicians on understanding and actual use of 5 A's model tool for tobacco cessation efforts during each patient encounter. The outcome results on the pre-and-post training survey revealed clinicians had improved awareness on each of the six questions related to use of the tool. Evaluation of chart audit group outcome data revealed 97.5% use of the 5 A's model following the implementation of the project versus 10% usage pre-implementation. These results show an increase in the smoking cessation screening rates using the 5 A's model tool. For the future, it is recommended that the 5A's model tool should be incorporated into the electronic health record (EHR) as a consistent form of assessment and intervention for smoking cessation efforts. The health and economic costs of tobacco use in military and veteran populations are high. In 2007, the Department of Veterans Affairs (VA) and the Department of Defense (DoD) requested that the Institute of Medicine (IOM) make recommendations on how to reduce tobacco initiation and encourage cessation in both military and veteran populations. In its 2009 report, Combating Tobacco in Military and Veteran Populations, the authoring committee concludes that to prevent tobacco initiation and encourage cessation, both DoD and VA should implement comprehensive tobacco-control programs.
