La prévalence des patients sous traitement anticoagulant augmente depuis plusieurs années, en particulier chez les patients âgés. Les anticoagulants, dont l'effet indésirable principal est l'hémorragie grave, sont en France au premier rang des médicaments responsables d'accidents iatrogéniques graves. L'objectif de notre étude est de décrire les patients de 80 ans ou plus, traités par anticoagulant, ayant présenté un accident hémorragique sévère pris en charge au CHLS du 1er janvier 2016 au 31 décembre 2017. Les patients inclus ont été identifiés par le Programme de Médicalisation des Systèmes d'Information aux moyens des codes CIM10 des troubles hémorragiques dus à des anticoagulants (D683) et des effets indésirables des anticoagulants au cours de leur usage thérapeutique (Y442). Les patients ayant présenté une hémorragie sévère (suivant la définition de l'HAS) ont été inclus, et plusieurs caractéristiques cliniques et biologiques ont été recherchées dans les dossiers médicaux informatiques, permettant l'analyse des facteurs de risques d'hémorragie, des différentes évaluations gériatriques, des modalités de traitements et du devenir. Les données étaient analysées par des tableaux croisés dynamiques via le logiciel Excel. Cent-dix patients ont été inclus sur les 303 dossiers extraits par le DIM. Soixante-cinq patients (59%) étaient traités par AVK et 20 patients (18%) par AOD. Les indications des traitements étaient dans 78,2% des cas pour la FA et dans 14,5% des cas pour une MTEV. Les hémorragies les plus retrouvées dans notre étude étaient les hémorragies digestives (39,1%), puis les hématomes musculaires (19,1%) et les hémorragies cérébrales (16,4%). Une forte prévalence de dénutrition, d'antécédent de chute et de polymédication était retrouvée, témoignant de la fragilité de notre population. Les modalités de prise en charge, notamment l'administration ou non de traitements antagonistes étaient variables. Les accidents hémorragiques étaient fatals pour 20 patients (18,2%) et la mortalité à 3 mois était élevée (26,4%), témoignant des lourdes conséquences des hémorragies graves dans cette population. La prescription d'anticoagulants chez les personnes âgées est donc à haut risque et nécessite une évaluation régulière de la balance bénéfice/risque. La mise en place et le suivi de protocoles pourra permettre une meilleure prise en charge des hémorragies graves.
