"The presence, or absence, of neglected tropical diseases (NTDs) can be seen as a proxy for poverty and for the success of interventions aimed at reducing poverty. Today, coverage of the public-health interventions recommended by the World Health Organization (WHO) against NTDs may be interpreted as a proxy for universal health coverage and shared prosperity - in short, a proxy for coverage against neglect. As the world's focus shifts from development to sustainable development, from poverty eradication to shared prosperity, and from disease-specific goals to universal health coverage, control of NTDs will assume an important role towards the target of achieving universal health coverage, including individual financial risk protection. Success in overcoming NTDs is a "litmus test" for universal health coverage against NTDs in endemic countries. The first WHO report on NTDs (2010) set the scene by presenting the evidence for how these interventions had produced results. The second report (2013) assessed the progress made in deploying them and detailed the obstacles to their implementation. This third report analyses for the first time the investments needed to achieve the scale up of implementation required to achieve the targets of the WHO Roadmap on NTDs and universal coverage against NTDs. INVESTING TO OVERCOME THE GLOBAL IMPACT OF NEGLECTED TROPICAL DISEASES presents an investment strategy for NTDs and analyses the specific investment case for prevention, control, elimination and eradication of 12 of the 17 NTDs. Such an analysis is justified following the adoption by the Sixty-sixth World Health Assembly in 2013 of resolution WHA6612 on neglected tropical diseases, which called for sufficient and predictable funding to achieve the Roadmap's targets and sustain control efforts. The report cautions, however, that it is wise investment and not investment alone that will yield success. The report registers progress and challenges and signals those that lie ahead. Climate change is expected to increase the spread of several vector-borne NTDs, notably dengue, transmission of which is directly influenced by temperature, rainfall, relative humidity and climate variability primarily through their effects on the vector. Investments in vector-borne diseases will avoid the potentially catastrophic expenditures associated with their control. The presence of NTDs will thereby signal an early warning system for climate-sensitive diseases. Neglected tropical diseases (NTDs) affect more than 1 billion people globally, causing serious, long-term health effects. Timely, effective interventions such as preventive chemotherapy, which involves the regular administration of safe, effective medicines to a defined population irrespective of its infection status, reduce morbidity and mortality. The new NTD road map advocates preventive chemotherapy as a core intervention to reduce the burden of NTDs. Effective administration of preventive chemotherapy relies largely on operational planning which addresses the need for programmes to reach all eligible people and maximize community demand for the intervention. Local, bottom-up planning (“microplanning”) fosters effective implementation and monitoring of public health programmes. Originally, public health implementers developed microplanning for use in vaccination campaigns, but various programmes, including those for control and elimination of NTDs, now use it. This manual is based on these and other country experiences in microplanning of preventive chemotherapy for NTDs. It describes the basic concepts of microplanning and provides steps and examples of tools to develop and implement a microplan. Since microplanning originates at the lowest level of implementation, and is a bottom-up process, the main target audience of this manual is individuals who are responsible for oversight of preventive chemotherapy against NTDs.

In addition, it presents the main facts or events that occurred in 2023. In line with the road map’s companion document Ending the neglect to attain the Sustainable Development Goals: a framework for monitoring and evaluating progress of the road map for neglected tropical diseases 2021−2030, the report includes quantitative information on the status of the overarching, cross-cutting and disease-specific indicators. This is followed by qualitative information on each of the three road map pillars and on regional and country progress. The conclusions of the report and way forward are further complemented by annexes on cross-cutting indicator 6, status of donated medicines for treatment of NTDs, articles on NTDs published in the Weekly Epidemiological Record, target product profiles published as of 31 December 2023 and the list of global NTD reports published by WHO so far.

Noting the success of strategies focused on morbidity control, the report shows how the spectrum of programme goals can now be broadened to include reductions in the prevalence and intensity of established infections and decreases in the intensity of transmission. Throughout, emphasis is placed on knowledge and experiences that can help programme managers establish feasible goals and then select control options in line with the form of infection, its public health importance, the degree and type of morbidity, available resources, and integration into the primary health care system. Information is specific to the different types of schistosomiasis and the distinctive epidemiological features, clinical manifestations, and response to treatment of each. The report has three main parts. The first, which is devoted to strategies for control, gives programme managers a concise, yet complete review of all factors that need to be considered when establishing priorities and deciding on the most appropriate options for control. Emphasis is placed on the many recent advances, including experiences with praziquantel, that have strengthened the tools available for prevention, diagnosis, treatment, and cure. While noting the severe financial constraints faced in many endemic countries, the report cites recent findings and experiences that make it possible for each endemic country to take action against schistosomiasis, even when resources are scarce and health services limited. The second part gives specialists a detailed state-of-the-art review of all technical developments relevant to control. Separate sections describe the distinctive patterns of morbidity and mortality seen in each form of infection, update knowledge about the parasite and its mammalian hosts, and summarize what is known about the snail intermediate host and its susceptibility to control by molluscicides, biological agents, and environmental management.
