Chronic venous insufficiency is the most common cause of lower extremity ulceration, accounting for up to 80 percent of the approximately 2.5 million leg ulcer cases in the United States. Venous ulcers are a result of damage to the valves present in the veins in lower extremities. They have a pattern of healing and recurrence. Traditional treatment method is using compression bandages for lower extremities. These venous leg ulcers are one of the most common clinical problem that nurses in community treat. Community health Nurses spent more time in assessment and treatment of these ulcers, patients' quality of life are also affected with the exudate, pain and compression bandages. Based on the various data collected and reviewed in the research, alternative to common approach of using four layer compression is discussed. Various articles and research materials were used to suggest the benefit of using two compression dressings over the four layer compression bandages. Benefit of using two layer compression dressings included reduced time spent in dressing changes, reduced frequency of dressing changes at home by nurses, reduction in exudate and pain. Overall patient quality of life noted to be better as compared when using four layer compression dressing. Duration for venous ulcers to heal were similar to that of two layer and four layer dressings. Strain on back of nurses when changing compression dressing was less.

This project evaluated the knowledge level of wound center nursing staff on venous leg ulcer etiology and treatments after an educational intervention. To determine the effectiveness of the educational intervention, participants completed a pretest and posttest evaluation. Venous leg ulcers (VLUs) are chronic lower extremity ulcers affecting older Americans. VLUs are painful and negatively affect activities of daily living. Inadequate VLU treatment can become an infectious process that can ultimately threaten limb existence. A quantitative, quasi-experimental study was conducted over three weeks in a hospital-based wound care center in central Florida. The theoretical framework applied to this project is Lydia Hall’s care cure and core nursing theory. There were seven participants evaluated in this study. Five of the nurses had a BSN degree as the highest level of education, while one was an APRN who had obtained an MSN. Two of the participants were licensed practical nurses. The nurses’ knowledge of VLUs was evaluated with a NetCE continuing education pretest and posttest. Before the posttest, participants completed a continuing education course. "Problem: Venous ulcer wounds are commonly managed in outpatient clinics by providers in conjunction with nurses; however, lack of wound care knowledge and evidenced-based wound care management among nurses make providing effective care challenging and difficult. Known as stasis ulcer, venous ulcer accounts for 80 percent of lower extremity ulcerations and its incidence increases with age and recurrence is quite common. These wounds can be challenging to treat, often taking at least four to six weeks to heal, and often requiring long-term therapeutics for optimal healing and reoccurrence prevention. Venous ulcer wounds are an important medical problem with substantial economic effects that can adversely impact patients’ quality of life and significantly increasing morbidity and financial burden. Intervention: Eligible primary care nurses viewed a venous ulcer wound care education video. The project focused on educating nurses on venous ulcer etiology and characteristics, wound care assessment, wound debridement and management, dressing selection and guidelines, and the critical principle of wound care. Measures: A pre- and posttest assessment measured any changes to knowledge and confidence in providing evidence-based venous ulcer wound care among primary care nurses. Data analysis was completed using a mean value, paired t-test, and ANOVA testing. Results & Conclusion: Overall, the mean value of all responses from the pre- to the posttest survey increased by 8.29%, indicating a positive impact of the intervention. Primary care nurse confidence in venous ulcer management had a paired t-test P-value of 0.0431. ANOVA tests were run to measure the correlation between years of nursing experience and average confidence level in providing venous ulcer wound care. The pretest analysis showed a statistically significant correlation between experience and confidence. However, this phenomenon disappeared upon analysis of posttest data. These results indicate that the education intervention was effective in increasing their overall confidence in providing venous ulcer wound care management, especially for nurses with fewer years of experience"--Abstract.
