Introduction : Les médecins, toutes disciplines confondues, ne seraient à l'origine que de moins de 5% des signalements d'enfants maltraités et paraissent ainsi en retrait face à ce problème grave et fréquent. Les médecins généralistes, qui assurent le suivi médical de nombreux enfants, ont un rôle central à jouer dans le dépistage et la prise en charge de l'enfance en danger. Notre travail a exploré le vécu et le ressenti de médecins généralistes face aux situations de maltraitance afin de comprendre les difficultés rencontrées et les freins au signalement. Matériel et Méthode : Etude qualitative par entretiens semi-dirigés auprès de 12 médecins généralistes Francs-Comtois Résultats : Parmi les freins au signalement identifiés on relève les difficultés d'identifications des situations de maltraitance. Celles-ci sont notamment liées à des difficultés pour définir la maltraitance et à l'influence des propres représentations des professionnels sur cette définition. Les médecins évoquent également leurs craintes du retentissement sur la relation médecin-malade, qu'ils considèrent comme fragile dans ces situations complexes, ainsi que la crainte des conséquences faisant suite au signalement. Si les médecins interrogés estiment avoir des lacunes au niveau du cadre juridique des procédures de signalement, ces lacunes ne sont pas citées comme des freins dans nos entretiens. Conclusion : Redonner aux médecins généralistes leur rôle central de médecin de famille dans ces situations et penser différemment la formation des étudiants parait essentiel. Des moyens simples et peu coûteux peuvent être mis en place: informer sur l'existence de médecins référents, fournir une fiche pratique d'aide à la rédaction d'une information préoccupante et d'un signalement, ainsi qu'une brève description des conséquences pratiques possibles de ces procédure. Notre travail invite à décentrer du cadre médico-légal la formation, et l'information, sur l'enfance en danger pour amener les praticiens et les futurs médecins à prendre conscience que leurs propres représentations influent sur leur définition de la maltraitance et sur leur capacité de dépistage.
