The special interest and importance, in a surgical sense, of the great European War lies not so much in the fact that examples of every form of gross lesion of organs and limbs have been seen, for if we read the older writers we find little in the moderns that is new in this respect but is to be found in the enormous mass of clinical material which has been presented to us and in the production of evidence sufficient to eliminate sources of error in determining important conclusions. For the first time also in any campaign the labours of the surgeon and the physician have had the aid of the bacteriologist, the pathologist, the physiologist, and indeed of every form of scientific assistance, in the solution of their respective problems. The clinician entered upon the great war armed with all the resources which the advances of fifty years had made available. If the surgical problems of modern war can be said not to differ sensibly from the campaigns of the past, the form in which they have been presented is certainly as different as are the methods of their solution. The achievements in the field of discovery of the chemist, the physicist and the biologist have given the military surgeon an advantage in diagnosis and treatment which was denied to his predecessors, and we are able to measure the effects of these advantages when we come to appraise the results which have been attained. Excerpt from Injuries and Diseases of Nerves: And Their Surgical Treatment It is now about nine years since, when House Surgeon at St. Bartholomew's Hospital, I first had the opportunity of treating some patients with wounded nerves. The cases interested me, for I found that but little was at that time known of the prognosis or clinical course of injuries of nerves. The treatment by suture was then in its infancy, and the results obtained by this method had not been ascertained. I therefore determined to try and obtain for myself the answers to problems which were not yet solved, and, as a result, the present work has gradually developed. In it are included the Jacksonian Prize Essay of the Royal College of Surgeons for the year 1882, and the Astley Cooper Prize Essay for the year 1886. The book may indeed be said to be merely a condensation of the latter essay, with the addition of such further knowledge as I have gained during the last three years. A portion of the following pages has also appeared in the Lancet for 1887, in the form of Lectures delivered at the royal College of Surgeons in that year. The work throughout has been done entirely from a clinical aspect, and whatever value it may possess is due to the fact that large numbers of patients suffering from nerve injuries of the most various kinds have been kept under constant observation for many years. I do not think that the practice of any one surgeon could have afforded a sufficient field for observation, but, fortunately for my purposes, I have been attached to a large hospital into which many and various accidents are yearly admitted, and in which my opportunities for clinical study have been practically inexhaustible. I am, of course, aware that there are excellent manuals on the subject by British authors, but none of them appears to me to cover the ground so fully, so authoritatively, and so originally as Dr. Tinel's. The continental clinic system makes it possible for the clinician to investigate a far larger number of cases than under our own individualistic methods. I hope that, with the return of peace, the clinic system, introduced in a modified form by my colleague, Mr. James Berry, at the Royal Free Hospital, will be continued and expanded. I have endeavoured to adhere closely to Dr. Tinel's text. If, however, I have failed to reproduce his meaning, the responsibility is certainly mine, as his book is most lucidlv written.
