Introduction : Le développement de l'implant cochléaire (IC) a permis à une proportion croissante d'enfants sourds d'entrer dans le monde de la communication orale. Bien que les bénéfices de l'implantation bilatérale soient maintenant bien connus, beaucoup d'enfants ne sont encore porteurs que d'un seul IC. L'objectif de cette étude est d'évaluer l'apport d'un deuxième IC posé de façon séquentielle et l'influence de certains facteurs sur ces résultats. Matériels et méthodes : li s'agit d'une étude rétrospective, monocentrique, sur 109 enfants ayant bénéficié de leur deuxième IC séquentiel entre 2008 et 2016. Les enfants étaient évalués avant leur implantation séquentielle puis à 3 mois, 1 an et 2 ans de l'implantation, à l'aide des tests d'orthophonie d'intelligibilité de la parole : SIR ; et de perception de la parole : CAP, reconnaissance de mots et de phrases dans le silence et dans le bruit. L'influence du délai entre les 2 IC et des performances obtenues avec le premier IC a été analysée. Résultats : L'IC séquentiel a permis une progression significative des résultats des tests d'orthophonie. Ces progrès étaient présents à la fois chez les enfants recevant leur deuxième IC de façon rapprochée mais aussi chez ceux avec un long délai entre leurs deux IC. Les patients considérés comme peu performants avec leur premier implant progressaient de façon significative après la pose de l'implant controlatéral. Conclusion : Devant les bénéfices qu'apportent l'IC bilatéral séquentiel, nous suggérons qu'un deuxième IC peut être proposé à tout enfant mono implanté, quelque soît le délai entre la pose des 2 IC et ses performances initiales.

Nous avons ainsi cherché à identifier en quoi la rééducation orthophonique post-second implant cochléaire se différencie de celle post-premier implant cochléaire, selon les orthophonistes et selon les patients. Nous supposions que l'introduction du second implant cochléaire chez l'adulte entraînait une réorganisation de la rééducation orthophonique, pour les deux acteurs de la rééducation. En effet, le nouvel implant nécessitant une deuxième réadaptation, les patients pourraient obtenir de nouveaux bénéfices et de nouvelles attentes par rapport à ceux obtenus pendant la 1ère. La pose du second implant cochléaire va par ailleurs engendrer des difficultés lors de la seconde rééducation, qu'il va falloir prendre en compte tant pour le thérapeute que pour le patient. Nous avons élaboré deux questionnaires, un à destination des patients et un pour les orthophonistes. Les patients sont des adultes, implantés séquentiellement avec un intervalle d'un an minimum entre les deux implantations et ayant suivi deux rééducations orthophoniques distinctes. Les orthophonistes ont suivi ce type de patients pour des rééducations post-premier implant cochléaire et post-second implant. Les patients ont rempli le questionnaire en version papier et l'ont renvoyé alors que les orthophonistes y ont répondu via une plateforme internet. Nous avons remarqué que la seconde rééducation est source de nouvelles attentes concernant surtout un meilleur confort auditif ainsi que des difficultés différentes de la première réadaptation, pour les patients et les orthophonistes. Elle apporte cependant des bénéfices au niveau de la localisation du son et de la qualité de vie (Dunn, Tyler & Gantz, 2012 ; King et al., 2014). Cette étude a permis de dresser un état des lieux des rééducations post-implantation dans le cas de l'implantation cochléaire bilatérale séquentielle chez l'adulte. Les données ressortant de ce travail permettront d'identifier des pistes dans l'élaboration du projet thérapeutique.

Several current linguistic approaches converge in rejecting the wide-spread idea that language is an autonomous system, i.e. that it is structured independently from the outside world and the natural equipment of language users. Around the world, semiotically biased linguistics (functionalism, naturalism, etc.) takes this position, which differentiates it very clearly from generative linguistics. This book brings together papers which address a range of issues regarding the nature and structure of sign languages and other gestural systems, and how they exploit the space in which they are conveyed. The chapters focus on five pertinent areas reflecting different, but related research topics: * space in language and gesture, * point of view and referential shift, * morphosyntax of verbs in ASL, * gestural systems and sign language, and * language acquisition and gesture. Sign languages and gestural systems are produced in physical space; they manipulate spatial contrasts for linguistic and communicative purposes. In addition to exploring the different functions of space, researchers discuss similarities and differences between visual-gestural systems -- established sign languages, pidgin sign language (International Sign), "homesign" systems developed by deaf children with no sign language input, novel gesture systems invented by hearing nonsigners, and the gesticulation that accompanies speech. The development of gesture and sign language in children is also examined in both hearing and deaf children, charting the emergence of gesture ("manual babbling"), its use as a prelinguistic communicative device, and its transformation into language-like systems in homesigners.
