Le médecin généraliste a un rôle important dans cette période, mais nous connaissons peu la consommation en soins primaires pendant cette période, notamment le délai entre sortie et première consultation de médecine générale. METHODE :Construction d’une base chronologique 2005/2008 des individus de 65 ans et plus avec le SNIIRAM (EPAS) comprenant : Hospitalisations, Consultations de médecine générale, Appariement avec ESPS, Création d’indicateurs temporels de parcours ,Description de la période post-hospitalisation (90 jours) :Délai entre sortie et première consultation de médecine générale, Présence de cette consultation (%) à J7 et J30, Réhospitalisation. Etude des facteurs associés à ce délai :Consommation médicale , Socio-démo-économiques. RESULTATS : La base comprenait 11407 individus (39.2% d’hommes), d’âge moyen de 74.9 ans, pour 13767 hospitalisations. Dans une période de 90 jours, le délai moyen entre sortie et consultation est de 18.15 jours ; 42.70% des hospitalisations sont suivies d’une consultation dans les 7 jours, 78.89% dans les 30 jours. La présence d’une consultation dans les 7 jours suivants la sortie n’était pas en lien avec le sexe, la couverture sociale, le renoncement aux soins déclaré, le niveau d’étude. Elle augmente avec l’âge et la présence d’une ALD. Les réhospitalisations précoces, dans les 7 et dans les 30 premiers jours, sont associées à un pourcentage plus faible de présence de cette consultation dans les 7 premiers jours. CONCLUSION : La consultation de médecine générale en post-hospitalisation est souvent rapide chez les personnes âgées ; son potentiel rôle réducteur d’événements indésirables nécessite des analyses approfondies.
