La reconstruction mammaire fait aujourd'hui partie intégrante du traitement du cancer du sein. Le lambeau de grand dorsal associé à une prothèse est une technique réalisée depuis près de 30 ans. Il s'agit d'un procédé fiable et reproductible entraînant des résultats cosmétiques satisfaisants à court et moyen termes. Cependant peu de publications étudient le devenir à long terme de ces reconstructions. Notre étude rétrospective présente une série de 47 reconstructions par lambeau de grand dorsal et prothèse, réalisées à l'hôpital Saint-Louis, avec un suivi minimum de 5 ans. Il apparaissait au terme de ce travail que d'une part, les séquelles de prélèvement n'étaient pas négligeables, et que d'autre part, le nombre de complications et de réinterventions liées aux implants était important. Le vieillissement du sein reconstruit diffère de celui du sein natif et une symétrie durable est difficile à obtenir. Nous nous orientons actuellement vers l'utilisation de techniques de reconstruction mammaire n'utilisant que les tissus de la patiente. Ces procédés semblent apporter des résultats plus stables dans le temps, mais il nous paraît nécessaire de leur appliquer la même évaluation de que celle réalisée dans cette étude avant d'étendre leur application.

Sujet : Depuis ces 20 dernières années, la reconstruction mammaire fait partie intégrante du traitement du cancer du sein, Je sein étant le symbole de la féminité. La reconstruction mammaire mixte associant lambeau de grand dorsal et implant a vu le jour à la fin des années 1970. C'est une techmque fiable et reproductible avec des résultats satisfaisants à moyen terme d'un point de vue qualité de vie et complications. L'objectif de cette étude rétrospective était d'évaluer les complications notamment les réinterventions liés à l'implant et celles au terme de la reconstruction, les résultats fonctionnels, esthétiques ainsi que la satisfaction des patientes sur le long terme. Avec le développement des techniques autologues pures, la technique mixte a-t-elle encore sa place dans la reconstruction mammaire? Y a-t-il encore des patientes susceptibles d'être éligible à cette techmque? Matériels et méthodes : Nous avons évalué de façon rétrospective dans notre service la satisfaction des patientes après reconstruction mammaire par lambeau de grand dorsal et prothèse par le questionnaire postopératoire du module« reconstruction» du BREAST-Q et un autre élaboré par nos soins, celui-ci s'attardant plus sur les séquelles du site donneur. L'évaluation fonctionnelle du membre supérieur homolatéral à la chirurgie était renseignée par le questionnaire DASH. La présence de complications entre les sites receveur et donneur était relevée ainsi que le pourcentage de réinterventions liées à l'implant. Résultats : De janvier 2003 à janvier 2015, 132 patientes ont été opérées ce qui représente 135 reconstructions mammaires. Dix décès étaient relevés, 84 patientes ont retourné les questionnaires (68,8% de participation). Au niveau de la cicatrice dorsale, 69 patientes (84, 1 % ) étaient au moins plutôt satisfaites du résultat. Le score moyen du DASH était 18,96. Le taux de complications globales était de 66% ; seulement 30,4% de celles-ci ont nécessité une réintervention avec un changement de prothèse chez 15 patientes (11,1%). Conclusion : Cette technique mixte a été supplanté par l'essor des lambeaux libres, épargnant le muscle et du transfert de graisse. Elle garde encore une place en reconstruction mammaire mais ne doit plus être proposée en première intention, seulement chez certaine patiente.
