Contexte : Le dispositif intra-utérin (DIU) est un moyen de contraception efficace et bien toléré. Il est cependant peu utilisé, en particulier chez les femmes dont le suivi gynécologique est assuré par le médecin généraliste. Le Nord-Pas-de-Calais est un bassin de population jeune et fertile. Le nombre des gynécologues diminuant, la prise en main de ce domaine et la pose du stérilet par les généralistes pourraient améliorer la prise en charge contraceptive des femmes. Méthode : Une étude qualitative par entretiens semi-dirigés a été menée de mars 2015 à février 2016 auprès de médecins généralistes installés dans la zone de proximité arrageoise. Résultats : Parmi les 15 médecins interrogés, beaucoup appréciaient le DIU pour son efficacité, sa tolérance et son observance mais seulement un le posait. Les freins principaux retrouvés étaient le manque de formation, le matériel nécessaire mentionné comme coûteux et difficile d'entretien, la peur des effets indésirables au moment de la pose et de l'engagement de la responsabilité personnelle. Le geste semblait inadapté à la pratique libérale car trop long. Enfin, d'après les généralistes, les patientes étaient réticentes au choix du DIU par crainte de ses effets-indésirables. Elles étaient également défavorables à la pose par le généraliste, considéré comme peu compétent dans ce domaine, trop proche affectivement ou du fait de son genre masculin. Les solutions proposées pour augmenter le nombre de pose étaient principalement axées sur les formations : initiale et continue, la planification de consultations dédiées et l'exposition de ce rôle possible du généraliste auprès des femmes. La féminisation de la profession et l'organisation en cabinet groupé pourraient aussi encourager les poses du DIU dans l'objectif d'une meilleure adaptation de la contraception et une plus grande satisfaction des femmes.

Les questions relatives à la santé sexuelle et reproductive sont reconnues comme un enjeu essentiel de santé publique. Tant la contraception que la contraception d'urgence ont connu en France un développement important, sans que les résultats en terme de prévention de grossesses non désirées ne soient à la hauteur de ce que l'on pourrait en attendre.Cette étude se propose de recueillir le point de vue des médecins généralistes sur la contraception d'urgence et plus précisément la contraception d'urgence mécanique via la pose d'un dispositif intra utérin. C'est une étude qualitative sur le territoire haut normand pour laquelle trois focus groupes ont été réalisés. Les médecins généralistes interrogés ont été surpris par la thématique dont certains ignoraient l'existence. Ils ont été séduits dans un premier temps par la théorie, la possibilité d'instaurer dans un même temps une contraception d'urgence et au long cours, ainsi que par des résultats supérieurs à ceux de la contraception d'urgence hormonale. Cependant beaucoup de freins à sa mise en pratique en médecine libérale sont ressortis tant d'un point de vue organisationnel, que culturel ou psychologique pour le praticien comme pour les patientes.

Contexte : A l'heure actuelle en France, le dispositif intra-utérin (DIU) est le deuxième moyen de contraception utilisé après la pilule et concerne environ 21 % des femmes de 15 à 49 ans ne souhaitant pas être enceintes. En dépit des recommandations de la Haute Autorité de Santé diffusées en 2004, précisant que le DIU pouvait être utilisé à tous les âges quelle que soit la parité de la femme, environ 1 % des nullipares y a aujourd'hui recours. Objectif : Etudier si les représentations des médecins constitutent un frein à la diffusion du DIU comme méthode contraceptive chez la nullipare. Matériel et méthode : Etude de pratiques. Enquête par auto-questionnaire réalisée dans le Doubs interrogeant un échantillon de médecins généralistes et l'ensemble des gynécologues. Résultats : Les infections et l'infertilité, la modification des saignements et les douleurs pelviennes font partie des complications et effets indésirables les plus redoutés par les médecins. Des problèmes liés à la pose sont soulevés par les médecins généralistes. Certaines situations socio-affectives de la femme ne rassurent pas. Le DIU est cependant dans l'ensemble considéré efficace, possédant une bonne réversibilité, sans interactions médicamenteuses et dénué de problèmes d'observance. Des représentations erronées, des lacunes dans les connaissances des médecins, un manque d'expérience de pose chez les médecins généralistes, des habitudes de pratiques sont autant de freins à la diffusion du DIU comme méthode contraceptive chez la nullipare. Conclusion : Une actualisation des connaissances des médecins, une formation plus " pratique " des médecins généralistes en gynécologie permettraient de diminuer leurs réticences vis à vis de l'utilisation du DIU chez la nullipare. Informée objectivement, la nullipare pourrait alors enrichir son panel contraceptif.

The Male Pill is the first book to reveal the history of hormonal contraceptives for men. Nelly Oudshoorn explains why it is that, although the technical feasibility of male contraceptives was demonstrated as early as the 1970s, there is, to date, no male pill. Ever since the idea of hormonal contraceptives for men was introduced, scientists, feminists, journalists, and pharmaceutical entrepreneurs have questioned whether men and women would accept a new male contraceptive if one were available. Providing a richly detailed examination of the cultural, scientific, and policy work around the male pill from the 1960s through the 1990s, Oudshoorn advances work at the intersection of gender studies and the sociology of technology. Oudshoorn emphasizes that the introduction of contraceptives for men depends to a great extent on changing ideas about reproductive responsibility. Initial interest in the male pill, she shows, came from outside the scientific community: from the governments of China and India, which were interested in population control, and from Western feminists, who wanted the responsibilities and health risks associated with contraception shared more equally between the sexes. She documents how in the 1970s, the World Health Organization took the lead in investigating male contraceptives by coordinating an unprecedented, worldwide research network. She chronicles how the search for a male pill required significant reorganization of drug-testing standards and protocols and of the family-planning infrastructure—including founding special clinics for men, creating separate spaces for men within existing clinics, enrolling new professionals, and defining new categories of patients.
