Simple et unanimement admise sur le plan conceptuel, la notion d'insuffisance rénale aiguë (IRA) est longtemps restée imprécise sur le plan opérationnel. En effet, plus de 30 définitions de l'IRA sont retrouvées dans la littérature. En fonction de la définition retenue, l'incidence et la mortalité varient dans de larges proportions, de 1 à 25% et de 15 à 60%, respectivement. Au-delà de ce problème de définition, la grande majorité des études ne fournit que des chiffres de mortalité brute et ne permet donc pas de savoir si les patients meurent d'IRA ou avec une IRA, en d'autres termes si l'IRA est en soi un facteur de risque de mortalité ou reflète simplement la gravité globale des patients. L'absence de définition consensuelle représentant un problème majeur, des experts en néphrologie et en réanimation ont développé une nouvelle classification proposée comme consensuelle, la classification RIFLE, de nature à faciliter la recherche épidémiologique dans le domaine de l'IRA. Cette classification semble bien corrélée avec la mortalité. En réanimation, cependant, on ne dispose que de très peu d'études de validation. L'objectif de l'étude servant de support à cette thèse est donc de tester plus avant la validité externe de la classification RIFLE sur une cohorte multicentrique française de patients de réanimation. L'IRA apparaît comme un facteur de risque indépendant de mortalité. La comparaison aux résultats antérieurs souligne toutefois les imperfections de la classification RIFLE et la nécessité d'une définition plus sensible qui permettrait une instauration plus précoce de l'épuration extra-rénale (EER). Les recherches futures devront déterminer les indications, la dose et la technique optimales de l'EER.

Contexte : L'insuffisance rénale aiguë (IRA) concerne plus de la moitié des patients de réanimation. L'IRA est source de progression de maladie rénale chronique (MRC). Chez les patients de réanimation, les données sur le pronostique rénal en fonction des caractéristiques de l'IRA sont limitées. En se basant sur la durée de l'IRA quelques études retrouvent des résultats contrastés concernant ce pronostic rénal. De plus, il n'y pas de donnée sur le pronostic rénal selon le mécanisme physiopathologique principal de l'IRA survenue en réanimation. C'est pourquoi nous avons construit l'étude MAKI pour décrire le pronostic rénal à long terme après une IRA en réanimation en fonction de sa durée et de son mécanisme physiopathologique principal. Méthodes : MAKI est une étude multicentrique, prospective et observationnelle. Le pronostic rénal d'une cohorte de 860 patients hospitalisés en réanimation plus de 24h sera évalué par le critère composite Major Adverse Kidney Events (MAKE). Ce critère MAKE sera évalué à 3 et 12 mois après le début de l'IRA ou de l'admission en réanimation si aucune IRA n'est survenue. Les patients seront classés en 3 groupes : 1) Les patients sans IRA durant leur séjour, 2) les patients ayant eu une IRA transitoire (récupération dans les 48h après le début de l'IRA), 3) les patients ayant eu une IRA persistante durant leur séjour.
