Il interférera dans son devenir, celui de sa famille, dans son vécu d'éventuelles nouvelles grossesses mais aussi et surtout dans la mise en place de la relation mère-enfant. La douleur obstétricale est un des principaux facteurs agissant sur le vécu de l'accouchement. Cette étude se penche sur qualité de la prise en charge de cette douleur en salle de naissance du CHU de Nantes. Après une recherche sur les différents moyens antalgiques disponibles, une étude prospective auprès de 73 patientes a été réalisée pour mesurer leur satisfaction. Les résultats montraient que malgré une prise en charge globale satisfaisante, des limites existaient et ce notamment en début de travail. En effet les alternatives à l'analgésie péridurale sont peu exploitées au CHU de Nantes et la prise en charge de la douleur obstétricale ne se résume bien souvent qu'à sa pose. Ce défaut d'alternative peut s'expliquer par le manque de moyens mais pas uniquement. La nalbuphine est par exemple une thérapeutique peu utilisée malgré sa disponibilité et la possibilité de sa prescription par les sages-femmes.

Introduction : A Nantes, environ 70% des accouchements se déroulent sous analgésie péridurale. Cette méthode de gestion de la douleur étant majoritairement utilisée, nous nous sommes intéressées à ce qu'en pensaient réellement les femmes. L'objectif de notre étude s'est basée sur la satisfaction maternelle concernant cette méthode d'analgésie, et les facteurs l'influençant. Matériel et méthodes : Nous avons réalisé une étude observationnelle, transversale, prospective et unicentrique. Elle s'est déroulée à la maternité du CHU de Nantes sur deux périodes : du 18 mai au 24 juin 2019 et du 9 au 21 aout 2019. La satisfaction maternelle a été évaluée grâce aux 218 questionnaires recueillis. Ils ont été distribués aux femmes ayant accouché par voie basse sous analgésie péridurale pendant leur séjour à la maternité. Nous nous sommes intéressées à la satisfaction globale, mais également à la satisfaction des différentes étapes de l'analgésie : informations données, insertion du cathéter, délai d'action, efforts expulsifs... Nous l'avons mesuré au moyen d'une échelle numérique allant de 0 à 10. Le seuil de satisfaction était fixé à la note de 7/10. D'autres données ont été extraites du questionnaire et du dossier informatique (données démographiques, déroulement du travail, intervention de l'équipe d'anesthésie...). Nous avons étudié grâce à une analyse multivariée, les différents facteurs pouvant influencer la satisfaction maternelle : âge, parité, IMC, PNP, appréhension de la douleur, mode de mise en travail, utilisation d'oxytocine, issue d'accouchement, durée sous APD. Résultats : La satisfaction générale obtient une moyenne de 8,2/10 et une médiane à 9, avec 84,5% de femmes donnant une note ≥ 7/10 ; les femmes venant accoucher à Nantes sont donc globalement satisfaites de cette méthode analgésique. Une femme sur cinq termine sa grossesse par césarienne dans le monde. Elle peut être réalisée pendant le travail ou avant, en urgence ou de façon programmée. La médecine évoluant continuellement, nous nous sommes intéressés à la technique de prise en charge de la douleur de ces patientes après une césarienne au CHU de Nantes. Celle-ci constitue de plus une priorité de santé publique. De nombreux articles qui encouragent la réhabilitation précoce en suites de couches montrent d'ailleurs que l'analgésie multimodale y prend une part importante. L'objectif de ce mémoire est d'observer les techniques de prise en charge de la douleur par les sages-femmes en suites de couches et d'en aborder les éventuelles difficultés. Nous avons pour cela recueilli 84 prescriptions postopératoires puis distribué 30 questionnaires aux sages-femmes que nous avons ensuite analysés. Les résultats principaux montrent que très peu de sages-femmes ont suivi une formation sur la prise en charge spécifique de la douleur.
