Les conséquences du syndrome du bébé secoué étant encore mal connues, nous avons décidé : 1) d'une part d'évaluer un des moyens de prévention actuelle : la plaquette du Conseil Général de Seine -Maritime. A travers une enquête menée auprès de 132 parents ou assistantes maternelles; nous avons essayé d'en évaluer la diffusion, le compréhension ainsi que les messages qu'ils en retenaient; 2) d'autre part , de rechercher les autres modes de prévention possibles et les lieux privilégiés pour le pérvention primaire. Par ailleurs, au niveau de la prévention secondaire, nous avons essayé d'établir un protocole simplie pour le suivi en médecine générale des bébés secoués. Méthodes : Nous avons fait une étude de type transversale descriptive auprès de 3 populations : les parents d'enfants de moins de 2 ans en cabinet de médecine générale, les parents d'un nouveau-né en maternité, et les assistantes maternelles. Cette enquête menée directement sur le terrain avec le support de la plaquette de prévention du bébé secoué s'est déroulée de mai 2005 à octobre 2005 en Seine-Maritime. Par ailleurs, nous nous sommes entretenus avec trois experts du syndrome du bébé secoué. Résultats : la diffusion de la plaquette est peu importante. La compréhension de cette plaquette est globalement bonne. Les message sont d'autant mieux retenus que la diffusion de la plaquette est accompagnée d'explications par un professionnel. D'autres modes de prévention ont été proposés par les personnes interrogées notamment audio-visuel. Les lieux de prévention privilégiés par les parents sont la maternité et le cabinet du médecin suivant l'enfant. Conclusion : Le prévention primaire peu utilement s'appuyer sur la plaquette, impérativement accompagnées d'explications médicales. D'autres modes de prévention pourraient être mis en oeuvre tel que : un court-métrage à diffuser dans les salles d'attentes, des réunions de sensibilisation des parents et des professionnels de la petite enfance. Une proposition de protocole de suivi en médecine générale du bébé secoué a été établie.

Le syndrome du bébé secoué est un traumatisme crânien infligé touchant majoritairement le nourrisson de moins de un an. Il survient à la suite d'un secouement réalisé par un adulte, dans le but de faire cesser les pleurs de l'enfant. Cette maltraitance spécifique, encore sous-évaluée, obtient un taux très élevé de mortalité et de morbidité. Toutefois de nombreuses études ont démontré l'intérêt, dans la diminution de son incidence, d'une prévention durant la grossesse et le post-partum. Nous avons donc souhaité évaluer la connaissance et la prévention du syndrome du bébé secoué réalisée par les sages-femmes en Alsace. Nous avons mené une étude transversale descriptive à l'aide de questionnaires distribués à 620 sages-femmes d'Alsace, tout exercice confondu, de septembre 2014 à décembre 2014. Parmi les 217 répondants, 45,2 % avaient de bonnes connaissances théoriques, 66,8% transmettaient une information systématique concernant les pleurs et 13% prévenaient systématiquement le syndrome du bébé secoué. Les résultats ont également permis de conclure sur l'influence de l'exercice professionnel sur la prévention. En effet les sages-femmes libérales et de Protection Maternelle et Infantile étaient celles qui prévenaient le plus le SBS, contrairement aux sages-femmes hospitalières et salariées du secteur privé qui expliquaient ce défaut de prévention par un manque de connaissances et de sensibilisation. Un besoin de réactualisation des savoirs et d'amélioration de la prévention a été mis en évidence. Pour y remédier, l'introduction de formations complémentaires serait souhaitable. De même, la création d'un protocole ainsi qu'un item dans le dossier de la patiente permettrait de parfaire la diffusion des informations en améliorant la coordination des différents professionnels de santé
