La dispensation du médicament et la sécurité du patient sont la priorité du pharmacien. Néanmoins, la iatrogénie médicamenteuse est un risque prépondérant dans le circuit aussi bien hospitalier que de ville, dont les conséquences sont susceptibles de mener à une hospitalisation. Premièrement, nous avons défini la iatrogénie et les mécanismes biologiques associés, puis nous avons présenté les différentes études déjà réalisées en France en guise d'état des lieux. Nous avons défini le rôle de la pharmacovigilance et rappelé les bonnes pratiques de dispensation, ainsi que les notions associées. Dans une seconde partie nous avons présenté, selon les résultats de notre étude, les médicaments les plus à risque d'effets indésirables graves (menant le patient à une hospitalisation/décès). Pour ainsi déterminer les moyens à mettre en oeuvre, autour de la prise en charge du patient, afin d'améliorer cette dernière et éviter ces effets. Ce travail montre que la communication entre professionnels de santé est importante. Tout comme les connaissances apportées aux patients, via les conseils du pharmacien d'officine, leur permettant de gérer au mieux leur(s) traitement(s) et leur santé

ADHD is a highly comorbid disorder in patients with substance use disorders (SUD). This comorbid patient population is, however, often undertreated. Diagnosing ADHD in patients with SUD is not easy due to the large overlap of the ADHD characteristics with symptoms of intoxication and withdrawal. These patients often disappear prematurely from the treatment services, on the one hand because their untreated ADHD symptoms make it more diffi cult to follow the therapy, on the other hand because the treatment programs do not take their limitations into account. An estimated 69000 people die each year from opioid overdose. Opioid overdose is easily reversed with the opioid antidote naloxone and with basic life support. Such care is generally only available in medical settings, however. These guidelines recommend that people who are likely to witness an opioid overdose, including people who use opioids, and their family and friends should be given access to naloxone and training in its use so that they can respond to opioid overdose in an emergency if a medical response is not available. Naloxone can be injected or administered intra-nasally and has minimal effects in people who have not used opioids. While naloxone administered by bystanders is a potentially life-saving emergency interim response to opioid overdose, it should not be seen as a replacement for comprehensive medical care.
