La dermatite atopique est une dermatose prurigineuse inflammatoire chronique alternant poussées et rémissions et liée à une sécheresse cutanée. Le traitement repose sur l'application quotidienne d'émollients et l'emploi de corticoïdes lors des poussées. Les immunosuppresseurs topiques avec le tacrolimus et le pimécrolimus, viennent enrichir l'arsenal thérapeutique. L'efficacité du tacrolimus est identique quelle que soit la zone traitée, ce qui ouvre la voie à son utilisation dans le traitement de la dermatite atopique dans les localisations (tête et cou) où les corticoïdes sont inappropriés. Le pimécrolimus, plus lipophile, a un passage systémique plus limité. Cette nouvel1e classe thérapeutique permet une alternative aux corticoïdes quand ils ne peuvent être utilisés du fait d'une localisation sensible ou d'un manque d'efficacité. Les immunosuppresseurs topiques restent des traitements d'exception en raison des effets potentiels à long terme.
