Introduction: L'IRM s'est imposée comme l'examen de référence du bilan d'extension de certaines pathologies, comme le cancer de la prostate, mais elle apparaît sous utilisée dans le cadre du cancer de la vessie. L'objectif de cette mise au point est d'identifié les évolutions récentes de cette technique pour le staging local et ganglionnaire des tumeurs vésicales ainsi que pour l'évaluation de la réponse aux traitements systémiques. Méthode: Une analyse bibliographique a été effectuée sur des articles des cinq dernières années intéressant l'utilisation de l'IRM dans le cancer de vessie à la fois au moment du diagnostic, de l'extension loco-régionale ou métastatique, dans le suivi post-thérapeutique, et dans l'évaluation du risque de récidive à distance. Résultats : L'association des séquences morphologiques et fonctionnelles notamment de diffusion (IRM dite « multiparamétrique ») apporte de nouveaux marqueurs de l'agressivité des lésions, et offre une sensibilité et une spécificité plus élevées qu'en scanner pour déterminer le caractère infiltrant, ou détecter certaines lésions planes ou de petite taille. Certains articles prouvent aussi son intérêt pour l'évaluation du risque de résistance à un traitement médicamenteux. Récemment, l'initiative VI-RADS a ouvert la voie à une standardisation des protocoles d'acquisition et de la description des lésions en radiologie pour faciliter la communication inter disciplinaire et développer l'utilisation rationnelle de l'imagerie dans la prise en charge ou l'évaluation du cancer de vessie. Conclusion : L'IRM multiparamétrique de la vessie est un examen d'imagerie utile et prometteur pour l'évaluation locale, locorégionale des carcinomes urothéliaux de vessie. Il devrait supplanter le scanner dans les années à venir du fait d'informations capitales à la fois diagnostiques et pronostiques améliorant une prise en charge « personnalisé » du patient ; l'établissement du score VI-RADS permettra d'homogénéiser les pratiques, de faciliter la coopération entre le radiologue, le clinicien et le chirurgien et facilitera la mise en place d'études à venir.
