The United States Government Accountability Office (GAO) and others have highlighted the need for an adequate and qualified nurse workforce to provide quality and timely care to veterans. The Department of Veterans Affairs' (VA) Veterans Health Administration (VHA) faces challenges such as increased competition for skilled clinicians in hard-to-fill occupations such as nurses. As GAO has previously reported, recruitment and retention is particularly difficult for nurses with advanced professional skills, knowledge, and experience, which is critical given veterans' needs for more complex specialized services. This book reviews the initiatives VHA has to recruit and retain its nurse workforce and the extent to which VHA oversees its nurse recruitment and retention initiatives. Furthermore, the book reviews the extent to which Department of Veterans Affairs medical centers (VAMC) have implemented VHA's nurse staffing methodology, and VHA oversees VAMCs' implementation and ongoing administration of the methodology.

The Veterans Health Administration (VHA) is America's largest integrated health care system, providing care at 1,243 health care facilities, including 172 medical centers and 1,063 outpatient sites of care of varying complexity, serving 9 million enrolled Veterans each year. In addition, VHA has opened outpatient clinics and established telemedicine and other services to accommodate a diverse veteran population and continues to cultivate ongoing medical research and innovation. Context: Nurse staffing is not constant across an entire hospital. Nursing care is delivered in geographically-based units, with wide variation in staffing levels. This is particularly true of intensive care compared with other acute care units. Objective: To evaluate the association of in-hospital patient mortality with registered nurse staffing and skill mix comparing hospital and unit level analysis using data from the Veterans Health Administration (VHA). Design, settings, and patients: A retrospective observational study using administrative data from 129,579 patients from 453 nursing units (171 ICU and 282 non-ICU) in 123 VHA hospitals. Methods: We used hierarchical multi-level regression models to adjust for patient, unit, and hospital characteristics, stratifying by whether or not patients had an ICU stay during admission. Main outcome measure: In-hospital mortality. Results: Of the 129,579 patients, mortality was 2.9% overall: 6.7% for patients with an ICU stay compared to 1.6% for those without. The significance of staffing depended on whether the analysis was done at the hospital or unit level. RN staffing was not significantly associated with in-hospital mortality for patients with an ICU stay (OR 1.02, 95% CI 0.99-1.03). For non-ICU patients, increased RN staffing was significantly associated with decreased mortality risk (OR 0.91, 0.86-0.96). RN education was not significantly associated with mortality. Conclusions: Our findings suggest that the association between RN staffing and skill mix and in-hospital patient mortality depends on whether the analysis is conducted at the hospital or unit level. Variable staffing on non-ICU units may significantly contribute to in-hospital mortality risk.
