Contexte : l'adolescence est une période de transition pendant laquelle il semble opportun de se soucier de la prévention, en particulier chez les adolescents entre 11 et 15 ans. La prévention et la promotion de la santé proposent des visions différentes dans ce domaine. De nombreuses études quantitatives ont été réalisées dans cette population montrant un bon état de santé de cette dernière. Le médecin généraliste en tant que médecin de premier recours se pose en interlocuteur ayant un rôle en prévention. Mais qu'en pensent les adolescents? L'objectif de cette étude est de comprendre les représentations et les attentes des adolescents âgés de 11 à 15 ans en prévention et promotion de la santé, et de faire quelques propositions dans une volonté d'amélioration des pratiques en médecine générale. Matériel et Méthode : une étude qualitative a été menée par 11 entretiens semi-dirigés et 1 focus group auprès d'adolescents âgés de 11 à 15 ans dans des centres socioculturels nantais. Une analyse thématique avec une part d'analyse structurale a été réalisée au fil de l'eau des entretiens, après retranscription complète de ceux-ci. Un focus group supplémentaire réalisé chez des 15-17 ans a permis une ouverture sur ces âges. Résultats : les adolescents rencontrés sont sensibles à l'aspect de promotion de la santé, permettant un échange sur leur quotidien et leurs pratiques réelles, en particulier concernant l'alimentation, le sport, le sommeil, l'hygiène et les soins de peau, ainsi que le bien-être psychique. Ils se sentent moins concernés par les consommations de tabac, et n'envisagent pas d'aborder la sexualité et les consommations de toxiques se sentant trop jeunes. Ils bénéficient déjà de nombreuses actions de prévention en santé par leurs sources principales que sont l'école et les médias en particulier la télévision, renforcées par l'expérience familiale et personnelle. Internet est très peu utilisé et méconnu concernant ce domaine. Leurs principaux interlocuteurs en santé sont les parents avec une implication émotionnelle ainsi que les amis et les pairs, dans un souci de comparaison. Ils ont peu de demandes en prévention en santé, et identifient peu le rôle du médecin généraliste dans ce domaine. La temporalité est spécifique et omniprésente concernant la prévention à ces âges. Discussion : des actions thématiques en prévention en cabinet de médecine générale ne semblent pas correspondre aux représentations ni aux attentes des adolescents en prévention en santé. Même s'ils expriment peu de demandes, des attentes ont pu être mises en évidence. Les adolescents ayant été très réceptifs à l'aspect de promotion de la santé lors de notre étude, nous proposons quelques axes de consultation dans ce sens, avec pour principal objectif l'identification par l'adolescent du rôle potentiel d'interlocuteur en prévention en santé du médecin généraliste, et l'ouverture à une vision positive de la santé.
