Depuis tout temps, l'homme a tenté de combattre la pathologie carieuse à l'origine des « maux) dentaires. A défaut d'empêcher efficacement son apparition, il est capable d'éviter sa propagation par l'éviction du tissu lésé. De ce fait, on est passé de l'utilisation d'instrument manuel tel que le trépan d'Archigène (100 ans après J.-c.) aux instruments à grande vitesse tel la turbine actuelle. Cette évolution concerne les portes instruments rotatifs mais elle va de pair avec celles des instruments rotatifs. Ces derniers sont de plus en plus capables, par leur constitution, de permettre d'éliminer sélectivement le tissu carié, tout en gardant le tissu dentaire sain pour une meilleure reconstitution. Nous aborderons dans un premier temps, l'histo-pathologie de la dent pour établir les bases des tissus sur lesquels agiront nos instruments. Dans un second temps, nous analyserons les différents types de cavités créées par nos outils et leur reconstitution avant de nous attarder longuement sur l'instrumentation proprement dite. L'avant dernière partie permettra de montrer les outils alternatifs à l'instrumentation conventionnelle pour finir par la présentation d'une séquence type d'instrument à utiliser face à une situation thérapeutique donnée.
