Souvent asymptomatique lors des phases initiales, il peut être découvert fortuitement voire même passer inaperçu. Il se développe surtout chez les sujets d'âge moyen et les femmes sont plus fréquemment touchées. Cliniquement, la forme réticulée du LPO est particuliGrement reconnaissable et pathognomonique. Cependant, les poussées d'activité successives, responsables de l'existence de nombreuses formes cliniques (érythémateuse, érosive, bulleuse, pemphigoïde, atrophique, nigricans, hyperkératosique ou hypertrophique) complique le diagnostic. Il repose donc sur un examen clinique rigoureux et une analyse anatomo-pathologique. Une surveillance est recommandée chez les patients présentant un LPO, notamment en raison du risque de transformation maligne. Le moindre doute devra conduire à l'orientation du patient vers un service spécialisé. Actuellement, l'absence de traitement curatif limite la prescription médicamenteuse aux LPO sévGres et douloureux. Le traitement de référence actuel est la corticothérapie locale. En raison du polymorphisme de cette maladie, il semble important de préciser les caractéristiques cliniques et histologiques de chaque forme afin de faciliter son diagnostic et sa prise en charge au cabinet dentaire.
