Nous avons souhaité étudier la santé des médecins généralistes sur le versant dépistage. Nous avons réalisé une étude qualitative auprès de 14 médecins généralistes volontaires installés en Haute-Savoie. Des entretiens semi-dirigés ont été menés pendant l'hiver 2014-2015 et retranscrits de manière anonyme. Les pratiques des médecins en matière de dépistage, pour leur propre suivi, sont plutôt conformes aux recommandations même si certaines sont perfectibles. Les médecins jugent leur état de santé au moins assez bon mais la majorité d'entre eux sont insatisfaits de leur suivi médical. Ils sont nombreux à être leur propre médecin traitant même si beaucoup jugent que ce n'est pas une bonne chose. Ils usent tous de l'auto-diagnostic et de l'auto-prescription. La souffrance psychologique est un problème fréquent et qui peut être lourd de conséquences. La plupart des médecins déclarent rencontrer des barrières à leur accès aux soins et avoir des difficultés à se confier en tant que patient. La relation thérapeutique entre un médecin malade et un confrère soignant est délicate. Il semble donc nécessaire de réfléchir à la création d'une structure de soins spécifique, dédiée à la prise en charge des médecins, avec des compétences préventives, diagnostiques, thérapeutiques et psychologiques ainsi que de former les professionnels de santé à la problématique du médecin malade

INTRODUCTION : Confronté à un environnement exigeant, le médecin estime qu'il est un patient comme les autres. Pour autant, il nie le plus souvent la fatigue et les symptômes annonciateurs de la maladie par indifférence, mépris de sa souffrance ou crainte de la vérité. Cette étude évaluait le ressenti des médecins généralistes libéraux quant au besoin de créer une structure qui serait dédiée à la prise en charge de leur santé. METHODE : Une enquête épidémiologique transversale descriptive déclarative a été menée à l'aide d'un questionnaire avec relances envoyé à 1076 médecins généralistes libéraux des départements des Savoie et de l'Isère entre mai et juillet 2012. RESULTATS : Le fort taux de réponse de notre étude (61,7%) montrait l'intérêt pour ce sujet. 68,9% étaient favorables a un tel système de soins. Les praticiens étaient demandeurs de consultations de prévention mais aussi de soins (prescription de thérapeutiques et d'actes) dans un cadre de confidentialité renforcée. Ils restaient attachés au libre choix du lieu et de leur médecin du centre. L'auto-surveillance (68,5%), le suivi déjà réalisé par d'autres confrères (30,2%) ainsi que le manque de temps disponible pour aller consulter (22,1%) semblaient être les principales causes de refus de cette structure. DISCUSSION : Malgré l'intérêt exprimé pour ce projet, l'adhésion risque d'être limitée par les freins sus cités. La difficulté est d'atteindre les plus négligents pour leur santé, ceux souffrant d'addiction ou de problèmes psychologiques. Le but d'une telle structure serait d'améliorer l'état de santé des médecins et par ce biais, d'assurer une prise en charge optimale des patients.
