Introduction- Le nombre d'enfants atteints d'insuffisance intestinale a augmenté au cours des dernières années. La nutrition parentérale à domicile (NPAD) représente le traitement de référence. La prise en charge des enfants en NPAD implique donc des changements importants pour les enfants et leur famille, ce qui peut affecter leur qualité de vie. À l'heure actuelle, il n'existe aucun instrument pédiatrique validé permettant d'évaluer la qualité de vie de ces enfants. L'objectif de cette étude est de sélectionner les éléments pertinents à inclure dans un outil d'évaluation de la qualité de vie des enfants en NPAD et de leur famille. Méthodes- Nous avons combiné une revue de la littérature puis une étude Delphi afin de sélectionner les items importants. Pour l'étude Delphi, les experts étaient des professionnels de santé ou des familles de patient, recrutés dans les centres Français prenant en charge des enfants en NPAD. Un seuil de 80% définissait le consensus. Les experts pouvaient proposer un ajout d'items lors du premier tour. Résultats- Une revue de la littérature nous a permis d'identifier et de soumettre aux experts 81 items, regroupant 12 domaines liés à la qualité de vie des enfants en NPAD et de leur famille. Trente-six experts de 7 centres ont participé au premier tour, et 23 au second. Quarante-sept items ont été validés au premier tour, 14 ajoutés entre le premier et le deuxième tour. Au total, 82 items ont été sélectionnés pour le questionnaire. Discussion- Cette étude nationale a permis d'identifier les items pertinents pour évaluer la qualité de vie des enfants atteints en NPAD et de leur famille. Un questionnaire provisoire a été élaboré et devra être testé et validé dans le cadre d'une étude future.

Nine years have passed since the first edition of Care of the Critically III Patient was published. As its editors Maurice Rapin and I were fortified by the many positive and helpful comments of reviewers and readers: the work appeared to have fulfilled our original aims and objectives. Intensive care medicine has, of course, advanced and changed in many ways since 1982 and it therefore seemed appropriate to consider the preparation of a second edition. Encouraged by the reception of the first and the timely prompting of the publishers we began, in 1986, to prepare for a new edition. Caring for critically ill patients is of universal concern and we were therefore delighted when Warren Zapol joined us as C'n editor widening our international base. The three of us subsequently spent many happy and productive times together planning the content, priorities, scale and contributions for the new edition. A great tragedy then overcame us: Maurice Rapin died suddenly. His death deprived us of a true friend and colleague who had been an international pioneer during the development of intensive care medicine. I will always remember, with affection and professional pride, the many hours he and I worked together in preparing the first edition of this book and also as founding editors of the now firmly established journal Intensive Care Medicine. An obituary, written by Warren Zapol, that appeared in the journal follows the Preface.
