La dénutrition résulte d'un déséquilibre entre les besoins et les apports énergétiques. En réanimation, elle augmente la morbi-mortalité des patients. L'ESPEN recommande l'utilisation de protocoles permettant d'identifier les patients à risque afin de leur prescrire un support nutritionnel adapté. Le support de prescription de nutrition parentérale utilisé actuellement par l'ensemble des services du CHU de Nantes n'est pas adapté au contexte particulier de la dénutrition en réanimation. Un nouveau support de prescription de nutrition artificielle a été créé par la pharmacie en collaboration avec les médecins réanimateurs et nutritionnistes. Ce nouveau support de prescription a été mis en place dans le service de réanimation chirurgicale polyvalente lors d'une étude observationnelle monocentrique de type avant/après dans le but d'évaluer les pratiques de nutrition artificielle du service et l'atteinte des objectifs caloriques et protéiques par les patients. Les résultats de cette étude montrent que les patients ayant bénéficié du nouveau support débutent la nutrition artificielle plus tôt et atteignent plus souvent les objectifs caloriques et protéiques respectivement à J3 et J7. Les résultats obtenus sont encourageants. Une nouvelle EPP sera réalisée dans plusieurs unités de réanimation au CHU de Nantes.

Childen's palliative care has developed rapidly as a discipline, as health care professionals recognize that the principles of adult palliative care may not always be applicable to children at the end of life. The unique needs of dying children are particularly evident across Africa, where the scale of the problem is overwhelming, and the figures so enormous that they are barely comprehensible: over 400,000 children in Africa died from AIDS in 2003, and out of the 166,000 children a year diagnosed with cancer, 85% of these are in the developing world. Despite the enormous need, provision of children's palliative care in Africa is almost non-existent, with very few health workers trained and confident to provide care for dying children. The challenges of providing palliative care in this setting are different to those in more developed countries, contending with the shortage of physical and human resources in addition to the vast scope of the care needed. L'industrie pharmaceutique a permis aux hôpitaux de disposer d'une large gamme de solutions nutritives permettant de répondre à tous les besoins (hypercatabolisme, instabilité métabolique, agression, pathologies digestives et rénales). La nutrition parentérale reste une méthode non physiologique d'entrée des nutriments dans l'organisme mais elle permet de se substituer à la nutrition entérale lorsque le tube digestif n'est pas pleinement fonctionnel (occlusion intestinale, surface absorbante intestinale insuffisante). De nombreuses études ont démontré son efficacité soit en tant que support nutritionnel exclusif soit de manière complémentaire à la nutrition entérale. La nutrition parentérale doit trouver sa place en second lieu de la nutrition entérale et son efficacité repose sur une bonne coordination entre les pharmaciens et les équipes référentes en support nutritionnel.
