L'ESPEN recommande l'utilisation de protocoles permettant d'identifier les patients à risque afin de leur prescrire un support nutritionnel adapté. Le support de prescription de nutrition parentérale utilisé actuellement par l'ensemble des services du CHU de Nantes n'est pas adapté au contexte particulier de la dénutrition en réanimation. Un nouveau support de prescription de nutrition artificielle a été créé par la pharmacie en collaboration avec les médecins réanimateurs et nutritionnistes. Ce nouveau support de prescription a été mis en place dans le service de réanimation chirurgicale polyvalente lors d'une étude observationnelle monocentrique de type avant/après dans le but d'évaluer les pratiques de nutrition artificielle du service et l'atteinte des objectifs caloriques et protéiques par les patients. Les résultats de cette étude montrent que les patients ayant bénéficié du nouveau support débutent la nutrition artificielle plus tôt et atteignent plus souvent les objectifs caloriques et protéiques respectivement à J3 et J7. Les résultats obtenus sont encourageants. Une nouvelle EPP sera réalisée dans plusieurs unités de réanimation au CHU de Nantes.
