Selon des études françaises et internationales, sa prévalence varierait de 10,1% à 55,2% en population générale. Le traitement de référence de la douleur chronique modéré à sévère reste les opiacés. La méthadone, opioïde de synthèse, n'a l'autorisation de mise sur le marché en France que dans le traitement substitutif des pharmacodépendances majeures aux opiacés dans le cadre d'une prise en charge médicale, sociale et psychologique. Quelle peut être son utilisation en alternative aux opiacés dans le cadre des douleurs chronique non cancéreuses en soins ambulatoires ? MATERIEL ET METHODE : une revue systématique de la littérature sur la méthadone comme traitement des douleurs chroniques non cancéreuses en soins primaires a été réalisée. Les banques de données utilisées était Pubmed, Cochrane et EM Prenium. RESULTATS : 34 articles ont été retenus selon les critères d'inclusion. D'après ces études, la méthadone est un traitement des douleurs chroniques non cancéreuses en seconde intention, après échec des morphiniques ou effets secondaires intolérables. La méthadone était initié à l'hôpital ou dans un centre antidouleur, puis le suivi se faisait en ambulatoire où les effets secondaires étaient connus et gérables. DISCUSSION : La méthadone est une alternative au traitement de référence dans la prise en charge ambulatoire de la douleur chronique non cancéreuse lorsque ce dernier est mal toléré. Cependant, ce sujet est peu étudié dans la littérature internationale et nécessite d'autres études, notamment avec des niveaux de preuve élevés.
