Objectifs : La région Grand Est a été l'une des plus touchées en France par la première vague de la pandémie COVID-19. L'objectif principal de cette étude rétrospective et monocentrique était de déterminer la prévalence de l'infection à SARS-CoV-2 parmi les enfants dépistés au laboratoire de Virologie des HUS par classe d'âge périscolaire (N1 :18 mois - 5 ans) et scolaire (N2 : 6 - 10 ans et N3 : 11 - 16 ans), à la fois en période pré-pandémique puis tout au long de la première vague pandémique. Il s'agissait également de caractériser le spectre de la maladie chez les patients COVID en population pédiatrique, d'un point de vue clinique mais aussi virologique et sérologique. Enfin, ce travail s'est intéressé aux modalités de transmission intra-familiale de l'infection. Patients et méthodes : En période prépandémique, les sérums d'enfants prélevés entre le 30/12/2019 et le 31/03/2020 pour sérologies virales, ont été testés à lai'de des trousses : Biosynex COVID-19 BSS (IgM et IgG anti-RBD protéine Spike (S)), Abbott Architect (IgG anti-nucléocapside), Euroimmun (IgA et IgG anti-S1 protéine S). Les sérums positifs d'enfants avec une histoire clinique compatible ont été testés par séroneutralisation à l'aide de pseudoparticules du virus (souche sauvage). Du 01/03/20 au 11/05/20, le dépistage par RT-PCR SARS-CoV-2 était réalisé à l'aide de la technique IP2 et IP4 de l'Institut Pasteur (gène RdRp). Des sérologies ont été réalisées à l'aide des trousses : Abbott Architect (IgG anti-S Quant II), Euroimmun (IgA et IgG anti-S1). Du 12/05 au 31/10/20, le dépistage par RT-PCR SARS-CoV-2 était réalisé à l'aide de la technique IP2 et IP4 ou d'un kit commercialisé par BGI (ORF1ab). Résultats: En période prépandémique : 75 enfants, répartis de façon équivalente entre les trois classes d'âge et selon le sexe, ont été testés. Le délai médian entre le prélèvement et la date d'apparition des symptômes était de 6 jours. Onze patients (14,7 %) ont présenté un test sérologique positif en IgA et/ou IgG anti-S. Deux enfants prélevés entre le 08/01 et le 16/01, présentaient des symptômes évocateurs, des index d'IgA très élevés (>20 S/Co), et des anticorps neutralisants contre le virus sauvage (2,0 et 2,9 log IC50).
