Le bilan d'extension des cancers invasifs du col utérin est dominé par l'évaluation de facteurs pronostiques essentiels : le volume tumoral, l'extension locale et ganglionnaire. Les limites du bilan d'extension par l'examen clinique, même pratiqué sous anesthésie générale, ont laissé une place de choix à l'imagerie par résonance magnétique qui, grâce à sa technique multi-plans et son haut pouvoir de résolution représente un outil diagnostique très performant. Notre étude a été rétrospective, menée sur 38 patientes âgées de 28 à 78 ans présentant un cancer invasif de stade > I A, ayant toutes eu un bilan d'extension pré-thérapeutique par IRM. Les données IRM ont été comparées aux résultats de l'examen clinique, de la cystoscopie, de la rectoscopie et enfin de la chirurgie pour les patientes opérées d'emblée. Les résultats de cette étude sont superposables à ceux de la littérature ; ils ont confirmé les performances de l'IRM dans le bilan pré-thérapeutique des CCU et montré les limites de l'examen clinique, même pratiqué sous anesthésie générale.

However, the recognition that prognostic factors often have a greater impact on outcome than available therapies and the proliferation of biochemical, molecular, and genetic markers have resulted in renewed interest in this field. The outcome in patients with cancer is determined by a combination of numerous factors. Presently, the most widely recognized are the extent of disease, histologic type of tumor, and treatment. It has been known for some time that additional factors also influence outcome. These include histologic grade, lymphatic or vascular invasion, mitotic index, performance status, symptoms, and most recently genetic and biochemical markers. It is the aim of this volume to compile those prognostic factors that have emerged as important determinants of outcome for tumors at various sites. This compilation represents the first phase of a more extensive process to integrate all prognostic factors in cancer to further enhance the prediction of outcome following treatment. Certain issues surround ing the assessment and reporting of prognostic factors are also considered. Importance of Prognostic Factors Prognostic factors in cancer often have an immense influence on outcome, while treatment often has a much weaker effect. For example, the influence of the presence of lymph node involvement on survival of patients with metastatic breast cancer is much greater than the effect of adjuvant treatment with tamoxifen in the same group of patients [5].

As with the introduction of x -ray computed tomography, much of the initial development of magnetic resonance applications tended to focus on the central nervous system. These guidelines have been developed to support countries to relieve pain in their paediatric populations. These guidelines address persisting pain in children caused by conditions such as cancer, HIV/AIDS, sickle-cell disease, burns, trauma, and phantom limb pain. Recommendations -- developed following a careful and transparent appraisal of available evidence -- are presented for the pharmacological treatment of mild, moderate and severe pain. The guidelines include chapters on the various systems used to classify pain and on the evaluation of pain, which reviews the available tools for routine pain measurement in children. Provision of sustainable pain relief within health care systems is covered in a separate chapter. Since morphine and other opioid analgesics -- required for the relief of moderate-to-severe pain in children -- are listed under the 1961 UN Single Convention on Narcotic Drugs, the new guidelines include an annex explaining the Convention's requirements for the handling and procurement of opioid analgesics for the relief of pain.
