L'insuffisance rénale aiguë (IRA) peut compliquer la prise en charge des patients porteurs d'un cancer solide en majorant la toxicité des traitements et le risque infectieux. L'objectif de cette étude était d'évaluer l'incidence de l'IRA chez les patients de réanimation atteints d'un cancer solide, d'en identifier les facteurs de risque et d'évaluer son retentissement sur la mortalité. Il s'agit d'une étude rétrospective menée de 2011 à 2015 dans le service de réanimation médicale de l'hôpital Saint-Louis (Paris). Deux cent quatre patients porteurs d'un cancer solide (métastatique dans 63% des cas) ont été inclus. L'incidence de l'IRA selon la classification KDIGO était de 56% (stade 1 : 21%, stade 2 : 10%, stade 3 : 25%) et 12% des patients ont été traités par épuration extra-rénale avec une mortalité hospitalière de 33%. La récupération rénale à 3 mois était inversement proportionnelle à la gravité de l'IRA. La survenue d'IRA était associée au score IGS2 (OR 1,05 IC95% [1,03 ; 1,08]), à la localisation abdominopelvienne du primitif (OR 2,66 IC95% [1,30 ; 5,47]) et à l'administration d'une chimiothérapie néphrotoxique (OR 4,12 IC95% [1,83 ; 9,27]). La mortalité en réanimation était de 15%, significativement associée au score IGS2 (OR 1,04 IC 95% [1,01 ; 1,07]) et à la protidémie à l'admission (OR=0,44 IC95% [0,23 ; 0,96]). La mortalité à trois mois était de 37%, associée au performans status 3-4 (OR 6,59 IC95% [2,42 ; 18]) et à la protidémie à l'admission (OR 0,60 IC95% [0,38 ; 0,94]). L'IRA était fréquente chez les patients porteurs d'un cancer solide admis en réanimation mais n'était pas associée indépendamment à une surmortalité à court et moyen terme.
