L'insuffisance cardiaque aiguë (ICA) est une pathologie grave avec une mortalité élevée et un taux très important de réhospitalisations. Il apparaît nécessaire de faire évoluer sa prise en charge afin d'améliorer le pronostic des patients tout en contrôlant les coûts et en préservant la qualité des soins délivrés. Nous proposons une analyse descriptive du parcours de soins, des caractéristiques générales et de la morbi-mortalité à 3 mois dans l'ICA. Matériel et Méthodes : Nous avons réalisé une étude observationnelle, prospective et monocentrique au CHU de Nantes, en incluant de manière consécutive les patients hospitalisés pour ICA dans les services de cardiologie au cours de deux périodes de 8 semaines. Les caractéristiques globales et des données spécifiques au parcours de soins étaient recueillies. Un suivi à 3 mois était réalisé. Notre critère d'évaluation principal était un critère combiné associant mortalité totale et réhospitalisation(s) non programmée(s) pour cause cardiovasculaire à 3 mois. Résultats : Le critère combiné mortalité totale ou réhospitalisation(s) non programmée(s) pour cause cardiovasculaire concernait 28,9 % des patients. L'insuffisance cardiaque est un véritable problème de santé publique, de plus en plus fréquente. C'est une maladie grave avec une lourde morbi-mortalité. Notre étude a porté sur les patients insuffisants cardiaques hospitalisés dans le service de médecine polyvalente au CHU de Nantes. Leur profil était bien loin de celui des patients inclus dans les grands essais thérapeutiques. La moyenne d'âge était de 82 ans. Les femmes y étaient majoritaires, avec de nombreuses co-morbidités associées. L'insuffisance cardiaque à fraction d'éjection conservée était présente chez 58% des patients. Nous avons mis en évidence, comme dans la plupart des études, une sous-prescription des traitements recommandés ; principalement les IEC et les β-bloquants. Nous avons également pu constater que l'échographie, examen clé, n'a pu être réalisée pour la totalité des patients hospitalisés. Lors de la prise en charge hospitalière de l'insuffisance cardiaque aiguë, 20% de nos patients ont présenté une variation de la créatininémie de plus de 30%, attribuée à un mésusage des diurétiques. Cette iatrogénie montre bien la fragilité de ce type de patient. Cependant l'âge ne doit pas être un frein dans la prise en charge diagnostique et thérapeutique. Il faut savoir évaluer les bénéfices et les risques. L'insuffisance cardiaque, requiert aujourd'hui une prise en charge multidisciplinaire. L'action d'un réseau, complémentaire du médecin généraliste et de l'hôpital, permettrait probablement une amélioration de la prise en charge globale avec une meilleure collaboration entre les différents intervenants.

Journalists, always very direct and in search of sensation, essentially asked me two questions on the occasion of this workshop: What were the goals of the meeting? With the improvement of diagnosis through the development of image techniques, didn't the contrast media already have their future behind them? Many answers were provided during the course of the workshop, and in order to best answer the journalists I proposed the following synopsis. 1. Since the 1979 Colorado Springs workshop organized by E. Lasser, progress has been so rapid and the newly available works so numerous that another meeting on an international level for the purpose of pre senting and discussing these advances appeared indispensable. Why not then in Europe and why not in Lyon? To expand on this progress, by 1981 the new contrast media with less-hyperosmolar molecules, still in the trial stage in 1979, were al most all available commercially for angiography, albeit at prohibitive prices. The advantages of these various media are becoming better known; moreover, in the wake of Lasser's work, our understanding of the pathophysiology of their noxious effects is also advancing rapidly owing to the use of models (for the target organs: heart, vessel wall, nervous system, kidney; and for the more general reactions: blood cells, coagulation, complement system, circulating enzymatic systems).
