Throughout Latin America, social medicine has been widely recognized for its critical perspectives on mainstream understandings of health and for its progressive policy achievements. Nevertheless, it has been an elusive subject: hard to define, with puzzling historical discontinuities and misconceptions about its origins. Drawing on a vast archive and with an ambitious narrative scope that transcends national borders, Eric D. Carter offers the first comprehensive intellectual and political history of the social medicine movement in Latin America, from the early twentieth century to the present day. While maintaining a consistent focus on health equity, social medicine has evolved with changing conditions in the region. Carter shows how it shaped early Latin American welfare states, declined with the dominance of midcentury technocratic health planning, resurged in the 1970s in solidarity against authoritarian regimes, and later resisted neoliberal reforms of the health sector. In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. How a coalition of Black health professions schools made health equity a national issue. Winner of the Phillis Wheatley Award by the Sons & Daughters of the United States Middle Passage Racism in the US health care system has been deliberately undermining Black health care professionals and exacerbating health disparities among Black Americans for centuries. These health disparities only became a mainstream issue on the agenda of US health leaders and policy makers because a group of health professions schools at Historically Black Colleges and Universities banded together to fight for health equity. We'll Fight It Out Here tells the story of how the Association of Minority Health Professions Schools (AMHPS) was founded by this coalition and the hard-won influence it built in American politics and health care. David Chanoff and Louis W. Sullivan, former secretary of health & human services, detail how the struggle for equity has been fought in the field of health care, where bias and disparities continue to be volatile national issues. Chanoff and Sullivan outline the history of Black health care, from pre-Emancipation to today, centering on the work of AMHPS, which brought to light health care inequities in 1983 and precipitated virtually all minority health care legislation since then. This book focuses on the achievement of health equity. It proposes pragmatic, feasible plans to greatly reduce them or perhaps eliminate them. Health is taken here in its most global sense, as defined by the WHO: "Health is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity", which includes the achievement of one's full career potential (eg: equality of opportunity for minorities to become public health professionals). In this book, the author address disparities existing in several populations to include (but not be limited to) low-income, racial and ethnic minorities, women, children, low literacy, disabled, GLBTI, as well as other groups who are victims of health disparities. While the needs of these populations are discussed openly and candidly, the author vigorously fights stereotyping of specific groups. The book will expose the institutionalised, personally-mediated and internalised mechanisms, which cause or perpetuate differences in opportunities, stresses and exposures as well as in health care access and quality. Currently, health disparities represent the most important challenge of public health. This book proposes to unveil often undetected, neglected aspects of health disparities embedded in all aspects of health and healthcare as well as medical research. Concepts are discussed based on critical appraisal of available evidence in current literature. Uncomfortable issues such as racism and discrimination in health and health care are discussed, respectfully but openly. These issues are never easy to talk about; however, being too comfortable often means very little change. Being uncomfortable can be a stimulus for change.
