This dissertation examined the relationship between PTSD symptom severity, social support, and dyadic adjustment among male heterosexual veterans who served in Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF) and their female civilian spouses (n = 10 couples). Elevated rates of PTSD, trauma exposure, and dyadic distress were found in this sample such that 80% of the veterans and 30% of the spouses met screening criteria for PTSD and over 90% of the veterans and spouses reported clinically significant levels of relationship distress. The veteran's PTSD symptom severity, particularly the symptoms of avoidance and emotional numbing, were significantly and negatively associated with the civilian spouse's dyadic adjustment. Surprisingly, group support was positively associated with PTSD symptom severity. These preliminary findings suggest that there is a clear need for an empirically validated couple's treatment for OEF/OIF veterans with PTSD and their spouses. Couples therapists can focus on decreasing the veteran's avoidance and numbing symptoms, particularly avoidance of emotional expression, as these symptoms prove significantly detrimental to the health of intimate relationships. Future researchers may want to investigate what to treat first in PTSD-positive couples therapy: the individual's PTSD, the spouse's psychopathology, or the couple's marital distress.

To serve the increasing numbers of individuals who have survived interpersonal and domestic violence, or as refugees, have sought asylum from political violence, armed conflict, or torture, Transforming the Legacy presents an innovative relationship-based and culturally informed couple therapy practice model that is grounded in a synthesis of psychological and social theories. This unique couple therapy model encompasses three phases of clinical practice: Phase I entails a process of establishing safety, stabilization, and a context for changing legacies of emotional, sexual, and/or physical abuse. Phase II guides reflection on the trauma narrative. The goal of phase III is to consolidate new perspectives, attitudes, and behaviors. Female veterans' combat exposure to trauma places them at risk for developing posttraumatic stress disorder (PTSD), which has consequences for healthful reintegration to family and civilian life. Previous research found that wives who provide continuous care to male veterans with PTSD experience symptoms of psychological distress; however, little research has been conducted on the influence of female veterans' PTSD on their intimate male partners' (IMPs) psychological well-being. A multivariate correlational design was used to examine the influence of female veteran PTSD on psychological distress and relationship quality in IMPs. The couples' adaption to traumatic stress model was used as the theoretical framework. The research questions examined (a) the difference between female veterans with and without PTSD on length of time in relationship, combat experience, total number of deployments, and IMP psychological distress and (b) the influence of female veterans' combat experience, PTSD, and IMP psychological distress on relationship quality. A sample of 71 IMPs between the ages of 18 and 65 provided survey research data on the variables of interest. Psychological distress, number of partner deployments, and length of time in relationship discriminated significantly between IMPS whose partners were diagnosed with PTSD, not diagnosed, or did not know about the PTSD diagnosis. The regression results revealed that the psychological distress of IMPs and number of partner deployments positively predicted relationship quality. Attention to female veterans and their families can contribute to increased retention of female service members in the Army and successful integration into family and civilian life.
