The Database and Expert Systems Applications - DEXA - conferences are dedi cated to providing an international forum for the presentation of applications in the database and expert systems field, for the exchange of ideas and experiences, and for defining requirements for the future systems in these fields. After the very promising DEXA 90 in Vienna, Austria, we hope to have successfully established wjth this year's DEXA 91 a stage where scientists from diverse fields interested in application-oriented research can present and discuss their work. This year there was a total of more than 250 submitted papers from 28 different countries, in all continents. Only 98 of the papers could be accepted. The collection of papers in these proceedings offers a cross-section of the issues facing the area of databases and expert systems, i.e., topics of basic research interest on one hand and questions occurring when developing applications on the other. Major credit for the success of the conference goes to all of our colleagues who submitted papers for consideration and to those who have organized and chaired the panel sessions. Many persons contributed numerous hours to organize this conference. The names of most of them will appear on the following pages. The Second European Conference on Artificial Intelligence in Medicine followed the successful meeting in Marseilles in 1987. As for AIME 87, the goal of AIME 89 was to promote scientific interchange within and between all subfields of AI in medicine, among researchers from all over the world, and especially from Europe. There were sessions on: knowledge elicidation and acquisition, architectures for medical knowledge-based systems, clinical applications, methodology, reasoning based on physiological models, and uncertainty. It is clear form the quality of papers presented, that the rate of development which took place between the Pavia meeting of 1985 and AIME 87 has been well maintained. With the launch of the European Community's exploratory programme in Advanced Informatics in Medicine in Europe, 1989 is clearly a very important year for this discipline. AIME 89 provided an important forum which demonstrated progress in some of the more difficult methodological problems, and advances in the application of these techniques to real-world medicine. This volume should be consulted by anyone who wishes to appreciate the state of the art in Medical AI in Europe.

Computer technology has impacted the practice of medicine in dramatic ways. Imaging techniques provide noninvasive tools which alter the di agnostic process. Sophisticated monitoring equipment presents new levels of detail for both patient management and research. In most of these tech nology applications, the computer is embedded in the device; its presence is transparent to the user. There is also a growing number of applications in which the health care provider directly interacts with a computer. In many cases, these appli cations are limited to administrative functions, e.g., office practice man agement, location of hospital patients, appointments, and scheduling. Nevertheless, there also are instances of patient care functions such as results reporting, decision support, surveillance, and reminders. This series, Computers and Medicine, will focus upon the direct use of information systems as it relates to the medical community. After twenty-five years of experimentation and experience, there are many tested applications which can be implemented economically using the current generation of computers. Moreover, the falling cost of computers suggests that there will be even more extensive use in the near future. Yet there is a gap between current practice and the state-of-the-art.

What follows is a sampler of work in knowledge acquisition. It comprises three technical papers and six guest editorials. The technical papers give an in-depth look at some of the important issues and current approaches in knowledge acquisition. The editorials were pro duced by authors who were basically invited to sound off. I've tried to group and order the contributions somewhat coherently. The following annotations emphasize the connections among the separate pieces. Buchanan's editorial starts on the theme of "Can machine learning offer anything to expert systems?" He emphasizes the practical goals of knowledge acquisition and the challenge of aiming for them. Lenat's editorial briefly describes experience in the development of CYC that straddles both fields. He outlines a two-phase development that relies on an engineering approach early on and aims for a crossover to more automated techniques as the size of the knowledge base increases. Bareiss, Porter, and Murray give the first technical paper. It comes from a laboratory of machine learning researchers who have taken an interest in supporting the development of knowledge bases, with an emphasis on how development changes with the growth of the knowledge base. The paper describes two systems. The first, Protos, adjusts the training it expects and the assistance it provides as its knowledge grows. The second, KI, is a system that helps integrate knowledge into an already very large knowledge base.
