Introduction : il est recommandé d'opérer les fractures de l'extrémité supérieure du fémur dans un délai maximum de 48h. Dans la cohorte amiénoise de 2017, le délai moyen était de 3,5 jours. Afin d'envisager une diminution de ce délai, il apparaissait essentiel d'identifier les facteurs pouvant entrer en jeu. Objectif : Nous nous sommes posés la question de savoir si certains facteurs pouvant influencer le délai de prise en charge au bloc étaient identifiés et pris en charge aux urgences. Matériel et méthode : il s'agissait d'une étude observationnelle rétrospective monocentrique menée de janvier 2021 à juillet 2022 au Centre Hospitalier Universitaire (CHU) d'Amiens-Picardie. L'objectif principal était d'identifier les facteurs divergents dans la prise en charge depuis l'admission aux urgences jusqu'à la prise en charge chirurgicale entre les patients opérés à moins de 48h de l'admission aux urgences et ceux opérés à plus de 48h. L'objectif secondaire était de quantifier l'influence des facteurs identifiés et modifiables afin de pouvoir proposer des mesures correctives. Résultats : 248 patients étaient inclus. Le délai opératoire (DO) moyen était de 58,6 heures. Les facteurs influançants significativement le DO étaient l'autonomie du patient (p=0,049), le nombre de médicaments (p=0,023), le type d'anticoagulants (p=0,0006), les antiagrégants plaquettaires (p=0,003), la période d'admission (p=0,025), la douleur (p=0,00066). Discussion/conclusion : même si nos résultats ne nous ont pas permis d'identifier de manière significative tous les facteurs influençants le délai opératoire, des tendances se dessinent. Les troubles de l'hémostase constituent le motif de report principal. Ces résultats méritent d'être confirmés par des études plus puissantes interventionnelles et prospectives. La mise en place d'un protocole de service permettrait d'uniformiser les prises en charge afin d'optimiser les délais opératoires de ces patients.
