The subject of the apallic syndrome is one which has long been familiar to me, although I have not personally studied it as deeply as I would have wished. I became acquainted with this syndrome long before the last war, when my neurosurgical colleague Hugh Cairns (1952), made his pioneer contribution under the term "akinetic mutism" . This was an ar resting title, but it was one which did not altogether satisfy some of his colleagues, includ ing myself. We found it difficult to suggest an alternative. That is one reason why I wel come the expression "apallic syndrome" . Forensic practice has forced me from time to time to consider rather more deeply this distressing syndrome, and to try and marshal my ideas in a form which would satisfy my colleagues in the legal profession. More than once I have been instructed to make a medico legal assessment of these unfortunate patients. The points which have concerned my lawyer friends have not been matters of diagnosis, or of morbid anatomy, or of etiology. The fac tual problem which has been put before me was to make some approximate assessment as to the expectation of life. Vague guess-work is unacceptable in such circumstances. What the lawyers require is a precise and dogmatic answer.

Societies around the world are concerned about dementia and the other forms of cognitive impairment that affect many older adults. We now know that brain changes typically begin years before people show symptoms, which suggests a window of opportunity to prevent or delay the onset of these conditions. Emerging evidence that the prevalence of dementia is declining in high-income countries offers hope that public health interventions will be effective in preventing or delaying cognitive impairments. Until recently, the research and clinical communities have focused primarily on understanding and treating these conditions after they have developed. Thus, the evidence base on how to prevent or delay these conditions has been limited at best, despite the many claims of success made in popular media and advertising. Today, however, a growing body of prevention research is emerging. La Maladie d'Alzheimer (MA) représente actuellement, et de plus en plus, un phénomène sociétal. En effet, il s'agit de la plus importante cause de démence chez la personne âgée. Parallèlement, en raison du vieillissement de la population, la prévalence de la maladie ne cesse de croître. On prévoit d'ailleurs (d'après les résultats de l'étude PAQUID) 1,2 millions de malades pour 2020. Face à la progression constante de cette pathologie, et devant le constat d'un retard de diagnostic particulièrement important (notamment en France), une révision des critères diagnostiques a récemment été proposée par Dubois et al. (2007), afin de pallier aux imperfections du DSM-IV et du NINCDS-ADRDA. Ces nouveaux critères associent des spécificités neuropsychologiques et la présence des biomarqueurs de la MA, dans le but d'optimiser les capacités diagnostiques de cette pathologie. Par ailleurs, œuvrer dans la perspective d'un diagnostic plus précoce semble essentiel pour la mise en place de soins adaptés. Certaines études, notamment celle d'Amieva et al. (2008) se sont donc intéressées aux marqueurs neuropsychologiques spécifiques de la maladie. Il semblerait que les épreuves de fluence verbale soient parmi les plus sensibles aux effets précoces de la MA, et ce bien avant le retentissement des troubles au quotidien: bien avant, donc, le stade de démence. Pour confirmer cette hypothèse, nous avons procédé à une comparaison des profils (quantitatifs et qualitatifs) de patients atteints de la MA au stade débutant à ceux de témoins âgés, lors d'épreuves de fluence verbale sémantique (animaux) et formelle (lettre P). Il semble effectivement se dégager un profil particulier pour les patients, mais presque uniquement en fluence sémantique, où la quasi-totalité des variables pennet de distinguer significativement nos deux groupes. La fluence formelle en revanche ne permet pas de telle distinction.

Patients with movement disorders nearly always present as outpatients but, as aptly recognized by Drs. Frucht and Fahn, this may include acute manifestations leading to emergency presentations, often in an emergency room setting, where they are very likely to be unrecognized and therefore poorly managed. The authors define an “emergency” movement disorder as one in which failure to promptly diagnose and treat may result in significant morbidity or mortality. However, they also stress the importance of certain “can’t miss” diagnoses such as Wilson’s disease, dopa-responsive dystonia, and Whipple’s disease in which delayed diagnosis in less emergent situations can lead to slowly evolving and often irreversible neurological damage with tragic consequences.
