This report assesses the clinical- and cost-effectiveness of CR programs and the potential policy and research implications for the health sector. Randomized controlled trials of CR programs with an exercise component were systematically reviewed in two groups: comprehensive care or exercise-only. The main outcome measures were total mortality and cardiac mortality. Economic studies using the same population and interventions were also systematically reviewed. CR programs that include exercise, both exercise-only and comprehensive care programs, have beneficial effects on cardiac mortality. However, with respect to total mortality, exercise-only programs show a statistically significant reduction, whereas the comprehensive care programs showed a trend in that direction. The literature reports that these programs are cost-effective and may reduce costs to the health care system, particularly when patients fully participate in maintaining the required level of exercise over the long term.

Because many patients reduce exercise following outpatient cardiac rehabilitation (CR), we developed an intervention to assist with the transition and evaluated its feasibility and preliminary efficacy using a one-group pretest–posttest design. Five CR patients were enrolled ~1 month prior to CR discharge and provided an activity tracker. Each week during CR they received a summary of their physical activity and steps. Following CR discharge, participants received an individualized report that included their physical activity and step history, information on specific features of the activity tracker, and encouraging messages from former CR patients for each of the next 6 weeks. Mixed model trajectory analyses were used to test the intervention effect separately for active minutes and steps modeling three study phases: pre-intervention (day activity tracking began to CR discharge), intervention (day following CR discharge to day when final report sent), and maintenance (day following the final report to ~1 month later). Activity tracking was successfully deployed and, with weekly reports following CR, may offset the usual decline in physical activity. When weekly reports ceased, a decline in steps/day occurred. A scaled-up intervention with a more rigorous study design with sufficient sample size can evaluate this approach further.

Abstract: Objective: To examine the effect of exercise-based cardiac rehabilitation program for secondary prevention of coronary heart disease on cardiac-related mortality, recurrent cardiovascular event and quality of life. Methods: All studies published between 1990 and 2013 in PubMed, and from 1980 to 2013 in EMBASE, which evaluated the effectiveness of exercise-based cardiac rehabilitation program for coronary heart disease. Using the specific keywords "Cardiac rehabilitation," "Coronary heart disease" OR "Ischemic heart disease" [MeSH], "Exercise" OR "Physical activities" AND "Quality of life" OR "Mortality" AND Cardiovascular events" were searched. A total of 7randomized controlled trials out of 5,051articles from PubMed and 117 articles from EMBASE were included in this systematic review. The primary outcome measures used in the included seven studies were HRQOL, restenosis, cardiac event, cardiac related mortality. Similar demographic and clinical characteristics of the subjects between the intervention and the control groups were recorded. The studies were from five countries. The average age of the subjects in the seven studies was 61years, the average half of them have history of myocardial infarction. Though there were discrepancies among the results generated in the included studies, the potential benefits of exercise-based cardiac rehabilitation could be seen. Results: Compared with the non-exercise-based cardiac rehabilitation, patients allocated to the exercise-based cardiac rehabilitation program had greater improvement in HRQOL and reduction of cardiac events. The result of reducing restenosis was inconsistent. The cardiac related mortality is not significant difference between exercise-based and non-exercise-based cardiac rehabilitation.
