De nombreux travaux ont fait état de l’épuisement professionnel des internes en médecine en France et à l’étranger. L’internat est en effet une période souvent considérée comme éprouvante pour les internes, un passage obligé et critique durant lequel l’étudiant doit pouvoir améliorer ses stratégies d’adaptation face au stress chronique. Ces nouvelles stratégies pourraient l’aider tout le long de son exercice professionnel. Si on peut encore améliorer les facteurs entrant en jeu dans cet épuisement, qu’ils soient liés à l’exercice médical en évolution, à son organisation ou qu’ils soient plus individuels, l’étudiant doit pouvoir repérer ce qu’il peut améliorer lui-même dans son mode de fonctionnement. Cela contribuerait à améliorer le bien-être de l’interne sans perdre de vue que l’épuisement professionnel a des conséquences néfastes sur la qualité des soins et la santé de ces médecins en devenir. L’étude descriptive transversale réalisée à Marseille à partir de 254 auto-questionnaires retrouve un épuisement émotionnel élevé pour 15% des internes. 36% ont un score de dépersonnalisation élevé et 48% un score d’accomplissement personnel bas. Pourtant, l’étude ne retrouve que 29% d’internes à se sentir menacés par le burnout et environ la moitié ne semble pas connaitre les conséquences de ce syndrome sur la santé mentale et physique des étudiants ou médecins atteints. Dans le cadre de la prévention, les internes plus sensibilisés et mieux informés sur ce syndrome devraient pouvoir repérer chez eux-mêmes des signes précoces de cet épuisement. Il est important qu’ils connaissent les facteurs prédisposant principaux rapportés par les études et pressentent les facteurs plus personnels liés à la gestion du stress.

The routinization process, i.e., how service practices in urban bureaucracies become part of "standard practice," is described by examining the life histories of six types of innovation: computer-assisted instruction, police computer systems, mobile intensive care units, closed circuit TV systems, breath testing for driver safety, and Jet-Axe (an explosive fire-fighting device). The life histories are analyzed in terms of the achievement of ten organizational events, conceptualized as "passages" (transitions to another organizational state) or "cycles" (survival over periodic events). The study emphasizes how these events are critical to the life history of an innovative practice. The stages in which routinization occurs and the conditions that lead to it are discussed, and several strategies that were found effective in promoting routinization are presented. The study suggests several steps that, if confirmed by further research, will allow policy officials to assess and influence routinization.
