Epidemiological and animal studies associate high levels of dietary fat with increased risk of sex hormone mediated cancer, such as breast cancer. A high intake of total fat and omega-6 fatty acids increases risk while omega-3 (n3) fatty acids are associated with risk reduction. Our proposal is testing the effect of dietary fat and fatty acids on sex hormone concentrations in post-menopausal women. The objectives are to evaluate 1) the effects of total fat and n3 intake on plasma and urinary sex hormone levels, 2) the relationship between plasma fatty acids and plasma and urinary sex hormones, and 3) the effects of total fat and n3 on the association between sex hormone concentrations and urinary prostaglandin E2 (PGE2). We are performing a randomized, Latin square-designed controlled feeding study testing High Fat, Low Fat, and Low Fat + n3 diets, each of 8 week duration. In order to determine the estrogenic effects of the diets, sex hormone endpoints will be measured reflecting availability, metabolism, and action. Plasma fatty acids fractions and urinary PGE2 will be measured to evaluate mechanistic effects. At present 14 women have been screened and 5 are currently enrolled in the trial. No data has yet been generated.

This book was inspired by a gatheringofscientists in Los Angeles in 1994 under the auspices of the UCLA Clinical Nutrition Research Unit which is funded by the National Cancer Institute to promote new research into nutrition and cancer prevention. This unit supports research integrating basic and metabolic/clinical investigations which examine observations from epidemiologic studies and their application to the prevention ofcommon forms ofcancer through nutritional intervention. There is a great deal ofinformation from epidemiologic, experimental and metabolic studies implicating elements ofthe diet as important in the development and progression of common forms ofcancer including breast cancer, colon cancer, prostate cancer, and uterine cancer. When these forms ofcancerareexaminedcarefully, it isclearthat they share anumber ofcommon etiologic factors related to dietary fat, lipids, and hormones. A human cancer is usually discovered at a point where it has formed a detectable mass. For many forms of cancer, this may require 10 to 15 years from the time when the cancer is first initiated. Nutritional efforts at prevention may delay the progression ofcancer to a detectable mass resulting in reduced incidence and may retard the clinical progression and metastatic spread ofcancer after its primary treatment.

Abstract: Background: Breast cancer is the most frequent female cancer in both developed and developing world which comprising 16% of all female cancer according to WHO GLOBOCAN 2008. The statistic from Hong Kong Cancer Registry reported that breast cancer is the third commonest cause of female death in Hong Kong. Breast cancer incidence varies remarkably among developed countries. The high dietary consumption of soy isoflavones has been hypothesized to explain the lower breast cancer incidence among women in Asian countries in observational studies, but whether soy isoflavones exert estrogenic or anti-estrogenic in breast tissue remains uncertain. Objective: This systematic review was to assess the effects of isoflavone-rich soy consumption on breast cancer risk in pre- and post-menopausal women Methods: Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines for conducting and reporting randomized controlled trials were followed. Data sources: A systematic review of randomized controlled trials was conducted through searching databases: MEDLINE, PubMed and Cochrane Library (2002 until March 2012). Keywords for electronic searches included: [(soy OR isoflavones) AND (breast cancer OR breast neoplasms)] limited study types to human & randomized controlled studies. Study selection: RCTs of the effects of isoflavones or supplement versus placebo or control diet among pre- and post-menopausal participants who were currently free from breast cancer. Outcome measurements: serum sex hormones and IGF profile, mammographic density and menstruation cycle length Results: 15 RCTs (1527 women) compared isoflavones with placebo or control diet for study duration ranged from 2 months to 2 years. No significant effect was found on serum sex hormones, IGF profile, mammographic density or menstrual cycle length. The effect of menstrual cycle on mammographic densities was noticed. Conclusion: The results of the systematic review did not support the hypothesis that short-term isoflavones exposure has an effect on modulating breast cancer risk. The effect of menstrual cycle on mammographic densities probably reflects the effect of hormonal changes. Null results did not necessarily contradict the inverse association between soy intake and breast cancer risk from the results of epidemiologic studies. The absence of conclusive data on the effects might be attributable to the insufficient exposure duration in the RCTs. Women at high risk for breast cancer who are experiencing menopausal symptoms of hot flashes are being recruited for our study in Boston and surrounding areas. Breast Health Clinics and Women's Clinics are being used to inform women about the study. Radio advertising has also been used to locate our study population which has been highly successful. We have currently recruited 80 out of the 100 being sought. Women with greater than or equal 5 symptoms per day are randomized, using a double-blind, cross-over study design, to receive either a placebo bar or soy supplement bar for 3 months with one month of wash-out between protocols. Two blood samples are taken at baseline and during the last week of each of the two 3 month study periods. Serum levels of estrone, estradiol, free-estradiol, estrone-sulfate, androstenedione, luteinizing and follicle stimulating hormone are measured. Subjects keep a daily symptoms diary for the entire seven month study. A sub-set of women (N=10) will have serum genistein and diadzien analyzed to determine if their levels match their Asian counterparts. A matched group of control women with less than or equal 2 hot flashes/day are currently being recruited. Fourteen have been recruited. This study proposes to determine: (1) if a dietary soy supplement bar (containing 45 mg of phytoestrogens) relieves menopausal hot flash symptoms compared to a placebo bar, (2) if a dietary soy supplement bar affects hormone levels, and (3) the relationship between menopausal symptoms and hormone levels.

The annual research conference for 1996 of the American Institute for Cancer Re search was again held at the Loews L'Enfant Plaza Hotel in Washington, DC, August 29 and 30. The topic for this, the seventh in the series, was "Dietary Fat and Cancer: Genetic and Molecular Mechanisms. " Two separate presentations were given as the conference overview. "Fat and Cancer: The Epidemiologic Evidence in Perspective" noted that die tary fat can be saturated, largely from animal or dairy sources, or mono- or polyunsatu rated, mostly from plant sources. Unlike animal fats, fish contain relatively high levels of protective omega-3 fatty acids. Although the hypothesis that dietary fat is associated with cancer is plausible, the mechanisms involved are reasonable, and many animal studies support the hypothesis, there are many obstacles in any direct extrapolation to humans, in cluding imprecise measures of dietary fat intake, variability in individual diets, and spe cies variations. Despite these limitations, there is a weak positive correlation between colon cancer and dietary fat intake, but with substantial differences for various ethnic groups. In the case of breast cancer, there is substantial variation among countries and eth nic groups, but the overall evidence indicated an association with fat in the diet. Epidemiologic studies of dietary fat and prostate cancer are more consistent and most show a positive relationship. However, it was not clear which types of dietary fat were im plicated in the effect.
